Abstract
Our social relationships – including between intimate partners, families, friends, and within communities - are essential to health. People with strong and positive connections to others tend to live healthier and longer lives. Yet, some relationships can be bad for our health, including those that are violent or that influence us to take risks.
Researchers in this programme seek to understand how social relationships and health are linked. The relationships team studies both positive and negative influences but are particularly interested in how we might make use of the positive influences to improve health. The research focuses particularly on groups at highest risk of poor health, perhaps because they are excluded from broader society, or living in areas with few resources. The team views relationships between people as just one part of a bigger system of influences on health.
The programme uses different approaches to study and improve health. This includes talking to people about their experiences; advanced statistical methods to analyse survey results; holding meetings with community members to understand local health issues and design solutions; studies to find out how well these solutions work; and supporting other researchers and communities to use the best solutions.
In doing so, the team constantly looks for ways to improve their methods and works with scientists internationally to strengthen understanding of social relationships in public health.

Technical Summary
Positive social connections are protective of health. Social relationships can be strongly supportive, but can also be a source of poor health and risk behaviour. Our aim is to progress understanding of mechanisms linking relationships to health and translate this knowledge into relationship-based solutions to key population health issues. We have three primary objectives:
- To uncover multilevel and modifiable relational mechanisms that shape, and are shaped by, experiences of wellbeing, health, risk, illness and inequality;
1. - To improve methods for discovery and translational public health research focused on relationships and health;
2. - To co-produce and evaluate relationship focused solutions to key public health priorities.
The programme conceptualises relationships as inherently complex, heterogeneous and influenced by the social context in which they are enacted. Our research is framed by complex systems thinking, in which social relationships are construed as interdependent elements within a connected whole. Our emphasis is on transdisciplinary action research, involving partnership with stakeholders throughout the research cycle, from problem framing, through measuring and describing, to identification and evaluation of potential solutions.
Our three complementary workstreams are structured around the levels at which social relationships operate; in dyads, small groups, social networks and communities.
The first focuses on intimate relationships and families. We aim to lead internationally on the public health conceptualisation and measurement of sexual wellbeing, and on the co-production and evaluation of complex interventions that theorise and measure relational mechanisms of change. This includes sex education, whole-school approaches to gender-based violence, and prison-based parenting programmes.
The second examines the distinct influences of social networks on health. We seek to advance methodological innovation in capturing and analysing multi-layer networks and using this to optimise network-based interventions. We will advance understanding of how evolving digital interactions have altered relational influences on adolescent health. We will also develop novel methods to assess change in social networks over the lifecourse, using causal mediation methods to assess relational elements of inequalities in healthy ageing.
The third focuses on social relationships in communities. We will use a syndemics lens to advance understanding of health inequalities in marginalised communities; we will engage network analysis to establish the most effective ways of harnessing formal network support in areas of high deprivation; and we will employ systems thinking methods to understand the complex causes of social isolation, working with stakeholders to identify whole-community and strengths-based ways to tackle it.

| Impact | This study used an systems implementation evaluation approach developed as part of the EPSRC/ESRC Impact Accelerator Account grant. This approach was applied to evaluate the Child and Family Healthy Weight pilot in three areas of Wales. The evaluation gave recommendations to each area in terms of how to enhance their implementation and reporting of the systems approach to healthy weight, and new tools and methods they could apply. The report was received positively by Public Health Wales, who advised us that the three local teams were working on implementing suggested changes. Public Health Wales have since invited us to undertake a further evaluation of the updated programmes, and a separate evaluation of Health Board Obesity Strategies across Wales. |

| Description | Progress toward establishing the effectiveness of biopsychosocial treatment for patients with sexual problems is limited by the lack of brief measurement tools assessing change across various domains of the treatment model. We developed and psychometrically validated a new clinical evaluation tool, the Sexual Function Evaluation Questionnaire (SFEQ) to meet this gap. The SFEQ combines into a single scale the best performing items from two instruments that were piloted in a UK sexual problems clinic (n = 486): the Natsal-SF Clinical Version and the National Sexual Outcomes Group 1 measure. Internal construct validity evidence from exploratory and confirmatory factor analyses supported a 16-item measure consisting of one overarching dimension of overall sexual function distributed along four subscales: problem distress, partner relationship, sex life, and sexual confidence. The measure had satisfactory configural, metric, and scalar invariance over time and across groups based on gender, ethnicity, and age. Correlations with patient depression and anxiety demonstrated external validity. Change in scores over the course of therapy varied as predicted, with greater improvement in younger patients and in areas more amenable to change via therapy (sexual confidence and problem distress). The SFEQ is a brief clinical tool with the potential to assess sexual function and evaluate the effectiveness of biopsychosocial treatment programs. |

| Description | This dataset was used for the paper "Face mask use during the COVID-19 pandemic: how risk perception, experience with COVID-19, and attitude towards government interact with country-wide policy stringency" forthcoming in BMC Public Health Abstract Background: During the 2020 COVID-19 pandemic, governments imposed numerous regulations to protect public health, particularly the (mandatory) use of face masks. However, the appropriateness and effectiveness of face mask regulations have been widely discussed, as is apparent from the divergent measures taken across and within countries over time, including mandating, recommending, and discouraging their use. In this study, we analyse how country-level policy stringency and individual-level predictors associate with face mask use during the early stages of the global COVID-19 pandemic. Method: First, we study how (self and other-related) risk perception, (direct and indirect) experience with COVID-19, attitude towards government and policy stringency shape face mask use. Second, we study whether there is an interaction between policy stringency and the individual-level variables. We conduct multilevel analyses exploiting variation in face mask regulations across countries and using data from approximately 7,000 students collected in the beginning of the pandemic (weeks 17 through 19 2020) Results: We show that policy stringency is strongly positively associated with face mask use. We find a positive association between self-related risk perception and mask use, but no relationship of mask use with experience with COVID-19 and attitudes towards government. However, in the interaction analyses, we find that government trust and perceived clarity of communication moderate the link between stringency and mask use, with positive government perceptions relating to higher use in countries with regulations and to lower use in countries without regulations. Conclusions: We highlight that those countries that aim for widespread use of face masks should set strict measures, stress self-related risks of COVID-19, and use clear communication. |

Eligible participants were resident in Britain, aged 18-59 years, and the sample included a boost of those aged 18-29. Questions covered participants' sexual behaviour, relationships, and SRH service use. Quotas and weighting were used to achieve a quasi-representative sample of the British general population. Participants meeting the criteria of interest and agreeing to recontact were selected for qualitative follow-up interviews. Comparisons were made with contemporaneous national probability surveys and Natsal-3 (2010-2012) (see SN 7799) to understand bias. Wave 2 was conducted March-April 2021, approximately one year after the start of Britain's first national lockdown. Data were collected using an online web-panel survey administered by Ipsos. The sample comprised a longitudinal sample of Wave 1 participants who had agreed to re-contact plus a sample of participants residing in Britain, aged 18-59, including a boost sample comprising people aged 18-29. In partnership with our CONUNDRUM collaborators, we anticipate pursuing further external funding for a co-developed digital intervention focused on increasing contraception/sexual health literacy among young people. |

| Description | The SEED study is a cluster randomised trial of a school-based public health intervention. We ticked the 'clinical trial' box because there was no option to tick for a cluster randomised trial of a public health intervention. The intervention is as follows: Data collected on emotional wellbeing of students is fed back to schools alongside benchmarked data and used to support reflective discussion sessions led by educational psychologists. This enables schools to make action plans at classroom level but also whole school level (interventions such as restorative justice). The focus of these interventions is particularly on managing transition from primary to secondary school. Trial results are currently being analysed. The report will be submitted mid April 2020. Scottish Government are interested in scaled up roll out. SEED has been developed and implemented in close partnership with Scottish government (for instance the Scottish Government is adopting questions from the SEED survey into the census to enable sharing across data sources/support consistency in what is collected) The intervention scoping, development and feasibility trial was funded by Scottish Government/CSO. | Description | The STASH website houses a set of curated sexual health resources designed to be shared between peers via social media. The original STASH website was designed for the NIHR funded STASH feasibility trial [Moore 14/182/14]. Since the end of the Trial, we have used funding from the Scottish Government to implement key changes based on the findings of the feasibility trial. Specifically, these were: flexibility to share on any social media platform (previous site was linked to Facebook), enhanced search function and organisation of resources to make them easier to find, an escape button, simplified log in; improved design for both mobile and computer. |

However, people were surprised that a husband could legally rape his wife until 1989 and that corn flakes were invented in the late 1800s to reduce masturbation! | Results and Impact | To understand stakeholder priorities for future research on adolescent sexuality, gender and sexual behaviour we carried out 7 online agenda setting workshops with 28 young people (aged 14 to 23), 4 parents/carers (2 women and 2 men) and 11 mental health researchers and practitioners. The workshops led to the identification of priority topics on gender/sexuality pertinent to young people's mental health. The workshops also explored perceived negative and positive links between adolescent mental health and sexuality. Priority topics from the workshops have been taken forward to focus groups with young people to design a measure of sexual wellbeing. |

Report was co-authored by the research team and young people from the CONUNDRUM Action Group. It was based on eight online workshops to understand young people's priorities for digital sexual health information and support in Scotland. Workshop participants (n=35) were recruited via youth organisations, and were diverse in gender, sexuality, ethnicity, SIMD. Workshops were co-facilitated by trained peer researchers (n=4 young people) from the CONUNDRUM Action Group, alongside two members of the CONUNDRUM research team. Peer researchers participated in three capacity-building sessions to strengthen their knowledge, skills and confidence relating to workshop design, facilitation, research ethics, and data management. This engagement work was prompted by requests from NHS health improvement staff and is intended to inform improvement of online sexual health resources. IMPACTS: NHS Health Improvement staff reported a change in practice on the basis of insights from this report (i.e. deciding not to go ahead in the short term with a planned course of action relating to digital resources as it was felt these no longer met young people's needs, and instead further work was needed). The report was shared by NHS stakeholders in a meeting with PHS National Incident Management team established to respond to rapid rise in gonorrhoea. CONUNDRUM team were invited to advise Public Health Scotland on their public engagement strategy for a current safer sex campaign aimed at young people to address increases in gonorrhoea in under 25s. CONUNDRUM was directly cited in a letter from the Deputy Chief Medical Officer (CMO) to all NHS Boards regarding the campaign. Report shared amongst stakeholders - helped to raise confidence in youth involvement in shaping policies and services: Stakeholders from NHS and SG report that CONUNDRUM Action has modelled ways to involve young people (e.g. "learning from Conundrum is now built in to fabric of thinking among NHS improvement work - has become reference point to influence all work with children and young people in sexual health"; "really useful as a bench mark for smaller pieces of involvement work). The animation was produced by an award winning production company (Media Coop, Glasgow), and co-designed with members of the CONUNDRUM Action Group, including young people, NHS staff and researchers. The aim of the animation is to sensitise health system stakeholders on the importance and barriers to young people's involvement. The animation was developed based on co-produced evidence from a CONUNDRUM study using participatory systems mapping to understand the system underpinning youth involvement in sexual health decision-making in Scotland. There were stalls from across the University, and as part of this we ran an event where members of the public could share and discuss their feelings around adolescent mental health in schools. Members of the public drew situations in school they found stressful, and discussed what others had drawn. This activity helped discussions between parents and young people about what they found stressful at school, and enabled us to share results from our research about the importance of context with adolescent mental health. There were stalls from across the University, and as part of this we ran an event where members of the public could share and discuss their feelings around adolescent mental health in schools. Members of the public drew situations in school they found stressful, and discussed what others had drawn. This activity helped discussions between parents and young people about what they found stressful at school, and enabled us to share results from our research about the importance of context with adolescent mental health. |

| Results and Impact | In March 2023, the Katawag project organised an end of project webinar to share key study findings and engage a wide range of stakeholders in the Philippines, in the UK and internationally. The webinar's objectives were to : a)Raise awareness among BPO industry decision?makers and public health organisations within the Philippines (and international parent companies) of how work environments can negatively affect mental health among employees b) Increase understanding of the potential for workplace/organisational solutions to mental health improvement; c) Showcase animated video and systems map briefing, and advocate for greater focus on workplace mental health interventions addressing systems?level factors and co?designed with employees; d) Stimulate conversations and develop new partnerships (with BPO industry, policymakers) to support development of future phases of intervention development and evaluation (e.g. a feasibility trial of the co?produced intervention). Panel members were national experts on mental health and well-being, including from the private sector and academia. The workshop was well attended with a diverse range of participants (54 participants). The study findings and the intervention approach were well received. The animated video and research brief were shared with participants. One of the company's taking part in project was keen to continue a future collaboration. A representative from a large global investment firm also attended webinar and contacted the research team with an offer to further collaborate on another project. If a next project takes place, these companies will be contacted. |

The event was advertised widely in the health board - to anyone who works or could have contact with YP in acute and community settings. The aim was to present study findings and recommendations, and discuss these in more detail in relation to their specific context. The session was also recorded, so that it could be shared with those not able to attend. Attendees were very interested in study findings and mentioned how they had already changed some of their processes at a sexual health clinic based on the study findings. They were also keen to get involved in future CONUNDRUM related activities. The presentation was done by Carolyn Blake in collaboration with one of the study commissioners from NHS Lanarkshire ( Jo Duffy). |

A press release designed with the journal (Plos ONE) led to interviews with several journalists (Sunday Times, Daily Mail, Independent) and coverage in a wide range of national and Scottish media. The story was covered on 'The Nine' (BBC Scotland News programme) including interview with our project partner at Rape Crisis. KM was interviewed on BBC Scotland. | Results and Impact | Approx. 145 people attended our CONUNDRUM project Webinar in 2021 where we shared the key findings from our study, as well as the recommendations. The panel was chaired by Nicky Coia (NHS Greater Glasgow and Clyde) and the panel was made from research commissioners (NHS Lothian and NHS Lanarkshire) as well as a youth think-thank and charity working with young people. A 45 min discussion session followed presentations, where the overall feedback was very positive and engaging. After the webinar, the research commissioners form the Scottish Government and NHS Health Boards mentioned they wanted to pursue a next research collaboration, following the findings from CONUNDRUM. |

The meeting also included presentations from six research teams funded through the network, who have developed models across a range of public health areas including food advertising and the food environment, tobacco control, mental health, urban development and cardiovascular disease. Meeting attendees welcomed the opportunity to meet others working in the field, and the meeting generated plans for taking forwards further agent-based modelling work in public health. |

The briefing was disseminated via the Natsal Study and SPHSU twitter accounts, with the initial tweet from Natsal receiving over 3600 impressions and 274 engagements, including being retweeted by national sexual health networks and organisations, including British Association for Sexual Health and HIV and HIV Prevention England. The briefing was subsequently included in the Faculty of Sexual and Reproductive Health's e-bulletin on SRH and HIV policy. |

The YAIs have been working with members of the SOCITS team at the University of Glasgow and also the Mental Health Foundation. Initial YAI meetings involved co-producing the interview schedules. Subsequently, the YAIs were involved in fieldwork shifts, conducting remote interviews from the Mental Health Foundation offices. At two further meetings the YAIs gave feedback about the design of the Agent Based model of interactions and stress in the simulated school and how that related to their own experiences. 28/09/23, 29/01/24 Two one-to-one sessions were held with Youth Advisor Investigators (YAIs) to validate the findings of the remote walking interviews that they had conducted as well as the pupil-teacher workshops. With the facilitator they co-produced a Jamboard to capture their reflections. 07/09/24, 8/09/24 A one-to-one session was held to get feedback on the survey questionnaire online, including the clarity of the wording and instructions, with suggestions for how it could be proved being noted and many implemented. Feedback was also received from two YAIs who could not attend the online session on 7 September 2023 via e-mail. 30/01/24 The YAIs were invited to do a test and sense check of the revised survey on their phones. Feedback was received via e-mail that considered how the survey would be understood by neurodivergent participants and changes integrated into the online survey platform. This ongoing engagement has had an invaluable influence on the direction of the study and fed into practical decision making on interview, survey and model design. The YAIs are currently working on producing blogs to talk about their experience of being Youth Advisor Investigators. |

These sessions were run so that the stakeholders could better understand the situations in which students (and teachers) have varied experiences. We identified challenging situations and locations (using a map), and facilitated discussion between different groups in the school, so that they can better understand challenges around mental health within their school. |

The event was held in partnership with The Young Women's Movement and The Scottish Youth Parliament. The panel consisted of KM and two young women representing a grassroots organisation called 'Bold Girls Ken'. A series of prepared questions were put to the panel and the audience then submitted questions of their own. The discussion focused on the extent to which current Sex and Relationships Education in schools adequately covered key issues in consent education. The audience was primarily comprised on senior school students. The event was filmed and is now available online. After the event KM was approached for advice by the NSPCC regarding advocacy strategies on Relationships and Sex education in Scottish schools. |

| Results and Impact | The Sexuality and Abortion Stigma Study (SASS) team ran an interationally-attended webinar on Wednesday 3rd March 2021 to mark the launch of their project website and research briefings. The SASS project re-examines qualitative data from a decade of studies on abortion across the UK. Drawing on the accounts of women who have sought abortion, and health professionals involved in provision, the project addresses abortion stigma, and considers how to move the conversation on from stigma to normalisation, both in academic and in more everyday terms. In the run up to International Women's Day 2021, this webinar showcased the SASS study's findings, considered in the current context of Covid-19 and ongoing shifts in abortion provision in the UK. The webinar was chaired by SASS Advisory Group member, and President of IPAS, Dr Anu Kumar. We gave introductory lessons on complexity science, agent-based models, and system dynamics using examples from the social sciences as well as public health and included exercises. Moreover, we taught the basics of both agent-based models and system dynamics in software programs typically used for academic purposes. The goal was to offer postgraduate students to learn about the opportunities of using the respective methods and give an introduction to each application. Several participants seemed inspired by the workshop and may use it in their future research. |

The presentation was based on a paper co-written by researchers in the Relationships Programme which discussed four key relational mechanisms: social networks, social support, interactional norms and intimacy and how these were disrupted by the pandemic. 240 people attended and there was an engaged discussion following the presentation. Kirstin was subsequently asked to give the presentation again to the Wellbeing team with Scottish Government. Participants commented how helpful the talk was in helping them think about the impact of COVID on different aspects of social relationships. | Results and Impact | The Natsal COVID study team, co-led by University of Glasgow, presented preliminary findings from wave one of their national survey on the impact of COVID on sexual and reproductive health in Britain. Over 500 sexual and reproductive health professionals registered for the event, including from across Europe and the US. Around 300 people attended live. The presentations highlighted strong age-related inequalities, with adverse outcomes more commonly experienced by younger people. The findings also underlined that although SRH services moved rapidly to reconfigure services, there were delays to access and unmet need affecting contraceptive use, cervical screening and STI diagnosis. A recording of the webinar is housed on the Natsal website and has been accessed by journalists (Women's Hour; Sunday Times) interested in writing up findings, as well as health providers and academics. |
