Abstract
Obesity levels in the UK represent a key public health issue, with 67% of its population living with overweight or obesity. People living with obesity are more likely to experience a range of health issues including heart conditions, and Type 2 diabetes. They are also more likely to be living in areas of high deprivation. Reducing obesity levels has been a public health priority in the UK for decades but we have not yet managed to achieve that goal. This is partly due to the range of factors that influence body weight. One key challenge facing people living with obesity is being able to afford a healthy, balanced diet. Nutritionally poor and energy-dense foods that are often ultra-processed, are cheaper and more readily available. To start to address this challenge, we need better evidence on how to support healthier food purchasing patterns to improve their health and wellbeing, while considering environmental impact and sustainability. Food insecurity is 'the state of being without reliable access to a sufficient quantity of affordable, nutritious food'. Families on low incomes are more likely to be food insecure and they spend a greater proportion (three quarters) of their monthly food budget in supermarkets. Supermarket promotions, advertising, and online product placement decisions can impact this group's access to healthy foods. Importantly, healthy diets also need to be sustainable in terms of greenhouse gas emissions, water consumption, and land use; described as the 'sustainability footprint'. Our research will bring together food insecure people living with obesity, consumers, retailers, policy makers, and academics to co-develop and test strategies that can support future transformative potential in the food system.
Our diverse team of academic experts in social science, applied health, obesity, and data science, will combine our knowledge of large-scale population data with an understanding of lived experiences of food shopping for people living with obesity and food to develop practical solutions to promote sustainable and healthier food choices in this group.
To achieve this, we have designed an innovative four-part project. Perspective: we will work with people living with obesity and food insecurity to understand the key issues facing them while shopping. We will also engage with the retail sector and policy makers to understand their perspectives too. This will identify limitations and barriers of current strategies and scope out future opportunities for our project to make sure our work remains relevant and useful. Big Data: we will use anonymous large-scale data (from >1.6 million shoppers) obtained from a national high-street supermarket (retailer) to understand what foods people buy, how healthy these purchases are, their sustainability footprints and how these choices vary across different household types including those on low income. This will help identify in- store changes that would encourage healthier and more sustainable food purchasing for people living with obesity and food insecurity. Solution Space: we will use the findings from the first two parts of this project to co-design new approaches and test these in-store and online assess their effect on healthier and sustainable food purchasing behaviours. We will also test and measure the effectiveness of these strategies in a group of people who are actively seeking to lose weight (MoreLife patient cohort) and living with food insecurity. This will help to identify strategies that can help transform supermarkets to promote healthier and more sustainable foods. Delivery: we will engage with food producers, food retailers, patient groups, policy makers, and charity group representatives to ensure our project is relevant and transformative. We will do this by sharing our findings with those groups, using webinars, social media, workshops, and research briefing notes.

| Description | In westernised countries, food insecurity (FI) is robustly associated with low diet quality, and obesity. Grocery stores are one promising arena for interventions to facilitate purchasing of healthier, more environmentally sustainable food. Our work has revealed that greater experiences of FI were directly associated with greater experiences of barriers from the food environment (e.g., price), food preparation practices, lower healthy diet knowledge and physical ill-health. Moreover, greater experiences of FI were indirectly associated with lower diet quality via mental ill-health and greater experiences of anticipated stigma associated with being food insecure. Grocery store interventions based on price/ incentivisation were ranked most helpful in supporting healthier, more environmentally sustainable purchasing. These findings highlight the challenges faced by people living with obesity (PLWO) and with greater experiences of FI when shopping for healthy and environmentally sustainable food. Findings also underscore the need for policy development relating to price and affordability at a population-level, and for policymakers and healthcare professionals to consider how to address mental health and how to minimise anticipated stigma experienced by this vulnerable group. Findings provide insights for evidence-based policy on the need for upstream changes within the wider food system to address the social determinants of health and support people living with obesity and food insecurity to eat healthier and more sustainable diets. |
| Exploitation Route | In this White Paper, we present the findings from two of the FIO Food project stakeholders' workshops that we ran at the beginning of 2024. We have co-produced knowledge across academics, stakeholders and consumers' lived experiences to formulate actions for policymakers and healthcare professionals. | Description | Narrative Impact The FIO-FOOD project, focused on addressing food insecurity among individuals living with obesity continues to generate societal and economic impacts since its initiation. Through research activities, co-production with research beneficiaries, publications, and policy influences, the project has contributed to enhancing public awareness, informing policy changes, and fostering collaborative initiatives that tackle the intersection of food insecurity and obesity in the retail environment. Economic and Societal Impacts: The FIO Food research Team has directly contributed to improving the understanding of food insecurity and obesity, which has critical implications for both public health and the economy. The project's insights into food purchasing behaviors, the effects of the cost-of-living crisis, and the barriers to accessing healthier, more sustainable food choices are crucial for policymakers. Our findings have informed national consultations, including contributions to the Good Food Nation Plan Consultation (Sep 2024) and submitting written evidence to House of Lords' "Recipe for Health" report (Oct 2024) which aim to create a more sustainable, inclusive food system. The societal impact is also visible in the work with local communities, such as the Challenge Poverty Week food collections, which brought awareness to food insecurity at the grassroots level. By contributing to high-impact events like the Andrew Carnegie Lecture (6 November 2024) the project has successfully engaged diverse stakeholders, from the general public to policymakers, fostering a dialogue about the importance of affordable, sustainable, and nutritious food for vulnerable populations. Public Service and Policy Influence: One of the key outcomes of the FIO-FOOD project is its direct contribution to national policy through collaborations with governmental bodies. The team's participation in influential reports such as DEFRA Food Insecurity Report (Dec 2024) supported placing the issue of food insecurity at the heart of policy discussions. The project has emphasised the need for co-production approaches and the importance of the inclusion of lived experience perspectives in the research narratives. These contributions are expected to drive reforms in food policy that promote equitable access to healthy food and reduce health disparities. Route to impact Our research is developed and delivered in partnership with individuals who have lived experience of obesity. Co-producing a research project means that researchers, practitioners, and the public work together, sharing power and responsibility throughout the entire process, from project design to knowledge generation. This collaborative approach ensures the research directly benefits the individuals and communities it aims to help. The impact of this co-production is evident in the positive outcomes for social beneficiaries-those whose lives are improved by the actions and initiatives stemming from this project. To share these findings, we have produced Policy and Practice briefs and frequent Newsletters distributed to over 100 stakeholders. In addition, we are collecting qualitative evidence of societal impact through written reports, testimonials, and case studies from patients and the public. By disseminating our results to policy teams across the devolved nations, we aim to increase the reach (the number and diversity of beneficiaries) and significance (the extent to which the research has influenced and improved beneficiaries' understanding, awareness, and well-being) throughout 2025. Academic Impact Academically, the FIO-FOOD project has led to the publication of influential research to advance the understanding of dietary inequalities. The project's work has generated multiple high-impact publications in journals like Appetite, Food Policy, and Nutrition Bulletin, with findings that are already shaping future research and policy in the area of food security and public health. Collaborating and forming research partnerships The team are working with ASDA, Morrisons, Sainsbury's Plc, and Tesco, who collectively represent 65% of UK supermarket sales, to understand the impact of the HFSS legislation restricting location placement of HFSS products (part of the DIO Food Project - AFS). The FIO Food team expanded the stakeholder network with national and local collaborators (e.g. Aberdeen City Council, NIHR: Health Determinants Research Collaboration) which fosters a placed-based, multidisciplinary approach to tackling food insecurity. Workshops and collaborative efforts have provided a platform for co-creating solutions and policies that address food insecurity within local communities and the wider UK food system. These partnerships have generated actionable insights that are expected to influence public service delivery and lead to long-term improvements in health outcomes and food accessibility. In conclusion, the FIO-FOOD project has made a profound contribution to both academic research and real-world policy, with a broad societal impact. It has empowered communities with lived experience of food insecurity and obesity, informed national policies, and informed public health practices, while also contributing to economic resilience through improved dietary choices and healthier food system. These efforts are focused on transforming the UK food system into a more sustainable, inclusive, and health-focused environment. |

One collection began at the Rowett Institute, where generous donations were sent to CFINE, bringing support to those in need. Photos from the events were posted on social media to increase the reach and raise awarness about food insecurity, the FIO Food project and Challenge Poverty Week campaign. |

The key inights were to: 1) COLLABORATE ACROSS THE SUPPLY CHAIN: Take collaborative and inclusive approaches to designing, implementing, and evaluating strategies for change that include all stakeholders in the supply chain. What can you do? Partner with suppliers to source ingredients that are both sustainable and nutritious. 2) ADOPT A WHOLE SYSTEMS APPROACH: Food systems are complex and dynamic, often unpredictable because of local, national or external geopolitical and environmental factors. What can you do? Work in partnership with multiple organisations to make the most of combined resources, expertise, and knowledge to deliver change. 3) INVEST IN RESEARCH AND LONG-TERM FUNDING: Support research that provides robust evidence for policy changes and helps avoid unintended consequences. The workshop was run by Prof Alex Johnstone and Prof Flora Douglas and had 31 attendees representing the Aberdeenshire consumers. Challenges related to the cost of living crisis were discussed and barriers in purchasing foods that would help to maintain a healthy diet. A vignette was developed based on discussion themes which was shared with the project team to provide insights for the intervention, as well as with project stakeholders. |

Content was related to the FIO Food project around 'Healthy People, Healthy Planet' theme which includes healthy eating and gets children thinking about sustainability/ environment (transport, food miles). The targeted audience were 240 pupils in 10 classes, from P1 to P7. The workshop sparked questions from pupils who were keen to learn about healthier and sustainable eating. |

| Results and Impact | Station House Media Unit (shmu), established as a charity in 2003, is one of the core cultural organisations in Aberdeen, and is at the forefront of Community Media development in Scotland. The organisation also supports other disadvantaged communities, both geographic and communities of interest across the region. The invited group of teenagers was given a tour: vertical farm, analytical, Human Nutrition Unit, and kitchen/ dining room and got the chance to engage with resources around healthy and sustainable diets. |

| Results and Impact | Overall, 29 teachers from primary (Cluny, Kittybrewster, Methlick, Strathburn, Uryside, Westpark) and secondary (Bridge of Don Academy, Bucksburn Academy) schools attended a CPD event at the Rowett Institute. Participants were presented with practical activities and nutrition resources they could use within their classrooms on display. The feedback was positive, with 85% of attendees rating the training as very useful or higher. Teachers enjoyed the mix of talks, the tour and the hands-on experience of the resources. 96% expressed they had been provided with good ideas on ways to engage pupils on the topics of healthy and sustainable food and over 80% stated they felt more confident to teach pupils about healthy and sustainable food following the event. Sharing information on these resources on social media and in future CPD events will be beneficial for teachers across Scotland. |

The session utilised insights from the FIO Food project to: triangulate stakeholder perspectives, identify practical solutions for improving access to healthier and more sustainable foods within the food system, empower foo-insecure individuals to make a healthier and more sustainable food choices. This workshop was a critical step in addressing food insecurity in Aberdeen City. It demonstrated the value of interdisciplinary collaboration and evidence-based approaches in tackling systemic issues affecting vulnerable populations. Continued efforts will focus on transforming insights into actionable outcomes to benefit the community. |
