Abstract
The rising prevalence of obesity, cardiometabolic disease, asthma, osteoporosis and neurodevelopmental disorders over recent decades cannot be fully explained by genetic or adult lifestyle factors. Increasing evidence suggests that early life exposure to environmental factors influences offspring health. According to the Developmental Origins of Health and Disease (DOHaD) hypothesis transient environmental exposures during critical periods of development (e.g. fetal and early infant phases of life) can elicit lifelong effects on offspring health. Epigenetic phenomena have been proposed as mechanisms by which detrimental fetal or early life exposures, such as suboptimal nutrition during pregnancy or early childhood, may affect offspring phenotype and risk of adverse health years after exposure. Maternal diet during pregnancy is a modifiable behaviour that could impact on maternal, neonatal and child health. However the specific dietary requirements during pregnancy for optimal fetal growth and development are unknown. Furthermore studies addressing the hypothesis of fetal programming have focused on maternal intake of single nutrients during pregnancy. Nutrients are not consumed in isolation, thus examining the combined effects of multiple dietary components reflecting the whole diet could have important public health implications. There is a clear need for primary prevention of obesity, related cardiometabolic conditions and other chronic non-communicable diseases based on the DOHaD paradigm. This project aims to advance the state-of-the-art by significantly expanding the knowledge base regarding interactions between biomarkers of diet, epigenetics and offspring health. Utilising biological samples and data from existing European longitudinal birth cohorts at the international forefront of lifecourse epidemiology we will investigate the complex relationships between maternal diet (defined by dietary quality and a novel index of dietary inflammatory potential), offspring health outcomes (including adiposity, bone, cardiometabolic, respiratory and neurodevelopmental health) and epigenetic patterns (DNA methylation) from birth throughout childhood. Better understanding of nutritional and epigenetic biomarkers of offspring health will afford opportunities to refine dietary exposure measures and to aid development of more effective evidence-based public health strategies with an emphasis on advocating a healthy diet in pre-pregnancy, pregnancy and early postnatal life, to reduce obesity, improve health and attenuate development of a range of adverse health outcomes in future generations.
Technical Summary

Planned Impact
Given the research focus, the potential long term health and socio-economic impacts may be significant considering the health service and societal costs associated with obesity alone, currently and for future generations. Our results will improve understanding of the relationships between maternal nutrition, pregnancy outcome and subsequent child health. For example improved dietary quality and/or reduced pro-inflammatory dietary status achieved through revision of existing guidelines or development of new guidelines for optimal nutrition in pregnancy or development of functional foods may lead to beneficial health effects on women of reproductive years and their offspring which may have long term effects on their health into adulthood.

Datasets created included followup of the participants at aged 4 and 7 years (also funded through separate Versus Arthritis grant). Followup included questionnaire data about child health, diet and physical activity, child anthropometry, DXA scans of child (and mum in 4 year followup), HR periperhal quantititative CT scans of the tibia (at age 7 years), blood samples collected where possible Epigenetic arrays (Illumina Infinium EPIC 850k array) on umbilical cord samples collected at birth. Pyrosequencing of the RXRA locus (a key locus in vitamin D signalling), with demonstration of differences in DNA methylation between the supplementation / placebo groups. Genotyping of mother- offspring pairs. Data currently held by MRC Lifecourse Epidemiology Unit, however may be used in consortia studies in future through application to the MRC Lifecourse Epidemiology Unit. |

Results and Impact | Press release relating to our publication - A study with families in Southampton found 47 percent of six-year-olds did not meet the recommended daily guidelines for physical activity. The study was carried out pre-pandemic by researchers at the universities of Southampton and Cambridge. All families who took part were involved in the ongoing Southampton Women's Survey. Children who are active are more likely to be active adults, reducing their risk of obesity and many lifestyle-related diseases later in life. They need to be active for their muscles and bones to develop properly. Being active also improves their ability to concentrate, boosts their mood and leads to better educational performance at school. Yet the results from this study, published in the Journal of Physical Activity & Health, suggest almost half of children are not active enough. |

Rates of childhood obesity are increasing worldwide. In the UK, nearly a quarter of under-fives are overweight or obese. This increases to more than a third by the time children start secondary school. Children who are obese are more likely to be obese adults, with long-term consequences for their health. Unhealthy diets are an important factor that contribute to this. Research, led by Southampton's Dr Sarah Crozier, has found children aged eight or nine were more likely to be obese if their mother had a poor diet during - and before - pregnancy. The research identifies these as critical times, when initiatives to reduce childhood obesity may be more effective. |

Results and Impact | Keith Godfrey was interviewed on BBC Radio News which was broadcast in the early evening and at lunchtime. During his interview he called on councils to do more to support new mothers to stay physically active. He led a study on the activity of 800 women in Southampton and found fewer than half were getting enough moderate to vigorous physical activity. |

The open day was extremely well attended and we had great interest in both the activities and also our research ongoing within the Unit, particularly among undergraduate students and their parents. People reported changes in their views and opinions and asked for further information. |

Developmental Origins of Health and Disease (DOHaD) explores the relationship between early life experiences and adult health. It highlights scientific and clinical advances since a first edition was published in 2006. There have been important breakthroughs in areas such as epigenetics and environmental influences. There is also a new focus on the preconception period and the significance of paternal health. |

Results and Impact | Press release - Southampton researchers will lead one of several new national UKRI research networks helping tackle challenges in ageing. The MyAge network will focus on pathways of muscle development, differentiation and decline. Over the course of the two-year project, scientists aim to build new directions and collaborations for future research. They will also develop a roadmap of changes that can help people live independently for longer. The 11 new national networks are funded by the Biotechnology and Biological Sciences Research Council (BBSRC) and Medical Research Council (MRC). MyAge is directed in Southampton by Professor Keith Godfrey, Professor Karen Lillycrop and Professor Peter JS Smith. It is also led by Professor Carolyn Greig (Birmingham), Dr Kambiz Alavian (Imperial College), and Dr Mathew Piasecki (Nottingham). |

The project, led by Jon Tobias, Professor of Rheumatology at Bristol Medical School, will involve a collaboration between researchers from the University of Southampton, as well as Bristol, Manchester, Aberdeen, Cardiff and Queensland Universities.
