Abstract

Obesity levels in the UK represent a key public health issue, with 67% of its population living with overweight or obesity. People living with obesity are more likely to experience a range of health issues including heart conditions, and Type 2 diabetes. They are also more likely to be living in areas of high deprivation. Reducing obesity levels has been a public health priority in the UK for decades but we have not yet managed to achieve that goal. This is partly due to the range of factors that influence body weight. One key challenge facing people living with obesity is being able to afford a healthy, balanced diet. Nutritionally poor and energy-dense foods that are often ultra-processed, are cheaper and more readily available. To start to address this challenge, we need better evidence on how to support healthier food purchasing patterns to improve their health and wellbeing, while considering environmental impact and sustainability. Food insecurity is 'the state of being without reliable access to a sufficient quantity of affordable, nutritious food'. Families on low incomes are more likely to be food insecure and they spend a greater proportion (three quarters) of their monthly food budget in supermarkets. Supermarket promotions, advertising, and online product placement decisions can impact this group's access to healthy foods. Importantly, healthy diets also need to be sustainable in terms of greenhouse gas emissions, water consumption, and land use; described as the 'sustainability footprint'. Our research will bring together food insecure people living with obesity, consumers, retailers, policy makers, and academics to co-develop and test strategies that can support future transformative potential in the food system.

Our diverse team of academic experts in social science, applied health, obesity, and data science, will combine our knowledge of large-scale population data with an understanding of lived experiences of food shopping for people living with obesity and food to develop practical solutions to promote sustainable and healthier food choices in this group.

To achieve this, we have designed an innovative four-part project. Perspective: we will work with people living with obesity and food insecurity to understand the key issues facing them while shopping. We will also engage with the retail sector and policy makers to understand their perspectives too. This will identify limitations and barriers of current strategies and scope out future opportunities for our project to make sure our work remains relevant and useful. Big Data: we will use anonymous large-scale data (from >1.6 million shoppers) obtained from a national high-street supermarket (retailer) to understand what foods people buy, how healthy these purchases are, their sustainability footprints and how these choices vary across different household types including those on low income. This will help identify in- store changes that would encourage healthier and more sustainable food purchasing for people living with obesity and food insecurity. Solution Space: we will use the findings from the first two parts of this project to co-design new approaches and test these in-store and online assess their effect on healthier and sustainable food purchasing behaviours. We will also test and measure the effectiveness of these strategies in a group of people who are actively seeking to lose weight (MoreLife patient cohort) and living with food insecurity. This will help to identify strategies that can help transform supermarkets to promote healthier and more sustainable foods. Delivery: we will engage with food producers, food retailers, patient groups, policy makers, and charity group representatives to ensure our project is relevant and transformative. We will do this by sharing our findings with those groups, using webinars, social media, workshops, and research briefing notes.

PI Contribution Academic support and collaboration in experimental trial design in the retail environment.
Collaborator Contribution Sainsbury's Supermarkets bring notable added value to this project. They will share anonymised loyalty card shopper records with the research team to help in studying food purchasing patterns over time, across different parts of the UK. They have around 1.7 million primary shoppers who are defined as low affluence (based on income, tenure, and savings). We will be collaborating with Sainsbury's so that we can tackle the cost-of-living crisis directly with those involved in the food system. We will also be utilising surveys, focus groups and initiating a series of interventions to test what works best to transition customers towards healthier baskets in 'real life'.
Impact This is a multidisciplinary collaboration including academic experts from nutrition, and psychology disciplines as well as retail representatives and public and patient advocates. One of the first outcomes will be sharing survey results from the survey on healthy diet and sustainable eating during the cost of living crisis among Sainsbury's customers.

Results and Impact Cafe MED is a series that takes place monthly from January to May in the Suttie Centre for Teaching and Learning in Healthcare, at the University of Aberdeen Foresterhill Health Campus. Professor Alex Johnstone (project lead) and Dr Kevin Deans provided insight into the current and emerging challenges associated with tackling the obesity crisis, particularly the link between obesity and the cost of living crisis and the solutions our food system must consider. The audience consisted mainly of medical students, but also staff and anyone interested in the topic.

Results and Impact The FIO Food Team together with Aberdeenshire Council hosted a PPI workshop on the 4th of October 2022 at the Rowett Institute, University of Aberdeen to discuss diet inequalities. The workshop was run by Prof Alex Johnstone and Prof Flora Douglas and had 31 attendees representing the Aberdeenshire consumers. Challenges related to the cost of living crisis were discussed and barriers in purchasing foods that would help to maintain a healthy diet. A vignette was developed based on discussion themes which was shared with the project team to provide insights for the intervention, as well as with project stakeholders.

Results and Impact Workshop event at Abbotswells Primary School on the 8th of February, as part of their health and wellbeing week. Content was related to the FIO Food project around 'Healthy People, Healthy Planet' theme which includes healthy eating and gets children thinking about sustainability/ environment (transport, food miles). The targeted audience were 240 pupils in 10 classes, from P1 to P7. The workshop sparked questions from pupils who were keen to learn about healthier and sustainable eating.

Results and Impact Station House Media Unit (shmu), established as a charity in 2003, is one of the core cultural organisations in Aberdeen, and is at the forefront of Community Media development in Scotland. The organisation also supports other disadvantaged communities, both geographic and communities of interest across the region. The invited group of teenagers was given a tour: vertical farm, analytical, Human Nutrition Unit, and kitchen/ dining room and got the chance to engage with resources around healthy and sustainable diets.

Results and Impact Overall, 29 teachers from primary (Cluny, Kittybrewster, Methlick, Strathburn, Uryside, Westpark) and secondary (Bridge of Don Academy, Bucksburn Academy) schools attended a CPD event at the Rowett Institute. Participants were presented with practical activities and nutrition resources they could use within their classrooms on display. The feedback was positive, with 85% of attendees rating the training as very useful or higher. Teachers enjoyed the mix of talks, the tour and the hands-on experience of the resources. 96% expressed they had been provided with good ideas on ways to engage pupils on the topics of healthy and sustainable food and over 80% stated they felt more confident to teach pupils about healthy and sustainable food following the event. Sharing information on these resources on social media and in future CPD events will be beneficial for teachers across Scotland.
