ALSPAC: A reference population for genetic & environmental epidemiology

Technical Summary

The Avon Longitudinal Study of Pregnancy & Childhood (ALSPAC) is a recent population-based study designed to understand the ways in which the physical and social environment interact, over time, with the genotype to affect health, behaviour and development. The ALSPAC design offers special advantages, the most important being - enrolment in early pregnancy, banking of DNA from the children and parents (permitting genetic transmission tests and transgenerational imprinting studies); diverse environmental measures both physical and psychological; one geographical base (permitting medical record and school linkage; environmental measures in the home, and clinics for direct examination); an intensely studied 10% subset - the Children in Focus (permitting detailed hands-on follow-up, validation of questionnaire data from the cohort, and practice for the 7-8 year examinations on the full cohort). The 14,893 enrolled pregnancies (EDD 1.4.91 - 31.12.92) represented about 85% of the eligible population and 14,138 children (13,995 mothers) are being followed principally by questionnaires completed by parents (227,151 returned of the 280,373 sent) with plans for a ‘physical examination at 7 years and a ‘psychological‘ one at 8 years. A study of this complexity is highly collaborative (including topic-specific international workshops) and the academic cohesion needed to fully exploit this valuable national resource would be enhanced by acceptance as a Co-operative Group.

Title Cell lines 
Description A lymphoblastoid cell line bank derived from study participants has been developed. Over 11,000 cell lines have been established and are available for research (total anticipated 16,000 on children, mothers and partners). The cell lines are proving to be an invaluable source of material for several studies. Expression analysis has been completed using cell lines from 1000 study children for whom genome-wide genotyping data is available. Samples are also being used for in vitro assays relating protein expression to individual genotypes. The cell lines are also used to replenish our DNA bank ensuring enough material is available for genome-wide genotyping studies. One aliquot of all transformed cells are stored at ECACC, Health Protection Agency, Porton Down in their secure cell line storage facility.

Description Detailed phenotyping is required to deepen our understanding of the biological mechanisms behind genetic associations. In addition, the impact of potentially modifiable risk factors on disease requires analytical frameworks that allow causal inference. We have described the characteristics of Recall-by-Genotype (RbG) as a study design aimed at addressing both these needs. We have considered the efficacy and practicality of the RbG approach, provide a catalogue of UK-based resources for such studies and present an online RbG study planner.

Title UK10K - population based whole genome sequencing 
Description This is the collection of raw data and results from the UK10K study. By studying and comparing the DNA of 4,000 people whose physical characteristics are well documented, the project aims to identify those changes that have no discernible effect and those that may be linked to a particular disease; by studying the changes within protein-coding areas of DNA that tell the body how to make proteins of 6,000 people with extreme health problems and comparing them with the first group, it is hoped to find only those changes in DNA that are responsible for the particular health problems observed.

PI Contribution The Phenome Centre Birmingham is a large metabolic phenotyping facility led by internationally-recognised metabolomics and clinical experts at the University of Birmingham, in collaboration with Birmingham Health Partners. The centre has been established as part of a UK Stratified Medicine initiative led by the Medical Research Council (MRC) to develop capacity and capability to perform large-scale metabolic phenotyping of the human population for stratified medicine. The centre will work towards improving healthy ageing and disease diagnosis and treatment to enhance patient outcome. The centre is providing a key collaborator in the collection of specific mass spectrometry data in experiments explicitly designed to explore the downstream implications of genetic mutations discovered in genomewide genetic association studies.

Impact 1. Promotion of CLOSER/NHS Digital work led to the Centre for Longitudinal studies changing their approach to onward sharing arrangements with NHS Digital and leading to a successful ammendment to their data sharing contracts. 2. Established contact with the Department for Education to discuss linkage to the Longitudinal Educational Outcome (LEO) dataset. This is furthering understanding of how a linkage arrangements can be designed and implemented.

PI Contribution The Cohorts and Longitudinal Studies Enhancement Resources (CLOSER) project aims to provide and document a range of key variables that have been harmonised across multiple UK birth cohorts to encourage cross-cohort research on longitudinal data sets and permit compatrisons across as well as within cohort studies. Due to the restrictions on using and holding ALSPAC data (particularly data obtained through linkage) it is our aim to derive standardised measures from education, income and social class variables that are harmonised with derived data from the other cohorts. These variables, once derived, will be deposited back to ALSPAC with accompanying documentation and will not specifically be used for research outputs at this point other than documenting summary statistics. As such, there are no exposure, outcome or confounding variables to outline. Documentation will be held by CLOSER and made readily available to researchers in an attempt to provide a suitable search platform for commonly requested variables and therefore encourage cross-cohort analysis of data.
