Abstract

Our social relationships – including between intimate partners, families, friends, and within communities - are essential to health. People with strong and positive connections to others tend to live healthier and longer lives. Yet, some relationships can be bad for our health, including those that are violent or that influence us to take risks.
Researchers in this programme seek to understand how social relationships and health are linked. The relationships team studies both positive and negative influences but are particularly interested in how we might make use of the positive influences to improve health. The research focuses particularly on groups at highest risk of poor health, perhaps because they are excluded from broader society, or living in areas with few resources. The team views relationships between people as just one part of a bigger system of influences on health.
The programme uses different approaches to study and improve health. This includes talking to people about their experiences; advanced statistical methods to analyse survey results; holding meetings with community members to understand local health issues and design solutions; studies to find out how well these solutions work; and supporting other researchers and communities to use the best solutions.
In doing so, the team constantly looks for ways to improve their methods and works with scientists internationally to strengthen understanding of social relationships in public health.

Technical Summary

Positive social connections are protective of health. Social relationships can be strongly supportive, but can also be a source of poor health and risk behaviour. Our aim is to progress understanding of mechanisms linking relationships to health and translate this knowledge into relationship-based solutions to key population health issues. We have three primary objectives:
- To uncover multilevel and modifiable relational mechanisms that shape, and are shaped by, experiences of wellbeing, health, risk, illness and inequality;
1. - To improve methods for discovery and translational public health research focused on relationships and health;
2. - To co-produce and evaluate relationship focused solutions to key public health priorities.
The programme conceptualises relationships as inherently complex, heterogeneous and influenced by the social context in which they are enacted. Our research is framed by complex systems thinking, in which social relationships are construed as interdependent elements within a connected whole. Our emphasis is on transdisciplinary action research, involving partnership with stakeholders throughout the research cycle, from problem framing, through measuring and describing, to identification and evaluation of potential solutions.

Our three complementary workstreams are structured around the levels at which social relationships operate; in dyads, small groups, social networks and communities.
The first focuses on intimate relationships and families. We aim to lead internationally on the public health conceptualisation and measurement of sexual wellbeing, and on the co-production and evaluation of complex interventions that theorise and measure relational mechanisms of change. This includes sex education, whole-school approaches to gender-based violence, and prison-based parenting programmes.
The second examines the distinct influences of social networks on health. We seek to advance methodological innovation in capturing and analysing multi-layer networks and using this to optimise network-based interventions. We will advance understanding of how evolving digital interactions have altered relational influences on adolescent health. We will also develop novel methods to assess change in social networks over the lifecourse, using causal mediation methods to assess relational elements of inequalities in healthy ageing.
The third focuses on social relationships in communities. We will use a syndemics lens to advance understanding of health inequalities in marginalised communities; we will engage network analysis to establish the most effective ways of harnessing formal network support in areas of high deprivation; and we will employ systems thinking methods to understand the complex causes of social isolation, working with stakeholders to identify whole-community and strengths-based ways to tackle it.

Impact Alumni of the course have had several successes and impacts, including: A 2021 alumnus successfully applied for an ESRC fellowship using social network analysis to look at mental health among people released from prison. Several 2022 alumni have been applying social network analysis methods to the whole systems approach to Obesity in Public Health Wales. Another 2022 alumnus aimed to direct their PhD funding on a whole systems approach to incorporate network methods.

Description The Natsal-COVID Wave 1 survey was conducted four months after the announcement of Britain's first national lockdown (23 March 2020), between 29 July and 10 August 2020. Wave 1 was an online web-panel survey administered by survey research company, Ipsos MORI. Eligible participants were resident in Britain, aged 18-59 years, and the sample included a boost of those aged 18-29. Questions covered participants' sexual behaviour, relationships, and SRH service use. Quotas and weighting were used to achieve a quasi-representative sample of the British general population. Participants meeting the criteria of interest and agreeing to recontact were selected for qualitative follow-up interviews. Comparisons were made with contemporaneous national probability surveys and Natsal-3 (2010-2012) (see SN 7799) to understand bias. Wave 2 was conducted March-April 2021, approximately one year after the start of Britain's first national lockdown. Data were collected using an online web-panel survey administered by Ipsos. The sample comprised a longitudinal sample of Wave 1 participants who had agreed to re-contact plus a sample of participants residing in Britain, aged 18-59, including a boost sample comprising people aged 18-29. Title Natsal COVID Wave 1 dataset 
Description The National Survey of Sexual Attitudes and Lifestyles COVID-19 study (Natsal-COVID) is a national quasi-representative web-based survey of 6,500 participants resident in Britain. The survey captures the impact of COVID on sexual behaviour, sexual and reproductive health service use and intimate relationships. Wave one of the survey captures change during the first four months of the survey. The dataset is deposited in the UK dataservice archive and is 'safeguarded'. The deposit comprises: a technical report, codebook and questionnaire as well as the datafile itself.

Description Completed collection of baseline and follow-up data for the Trial of Healthy Relationship Initiatives for the Very Early-years (THRIVE). We recruited 500 women with additional social and care needs during pregnancy. Baseline data was collected between week 12 and week 27 of pregnancy. Follow-up data was collected when their infant was between 6 and 12 months of age.

The original CONUNDRUM study was co-developed with these collaborators and others in the Scottish sexual health community and involved over 2000 young people living across Scotland. Since the study ended, the team has continued to collaborate on ways to take forward the co-produced policy recommendations. This has included securing ~£60k additional funding for a new project ("CONUNDRUM Action") focused on increasing young people's meaningful involvement in sexual health policymaking within Scotland. This project has backing from the Scottish Government SHBBV team, who anticipate this engagement work informing the upcoming review of their SHBBV Framework. In partnership with our CONUNDRUM collaborators, we anticipate pursuing further external funding for a co-developed digital intervention focused on increasing contraception/sexual health literacy among young people.

One of the main aims of the group was to show from a workable example the benefits of meaningfully involving young people in the sexual health policymaking process.
Collaborator Contribution All partners have been engaged throughout the project and we have seen consistently high attendance in online Action Group meetings. In evaluation activities, Action Group members have reported a variety of impacts derived from their participation in the Action Group, including development of new collaborative cross-sector relationships, strengthening of existing partnerships, development of new skills and knowledge relating to youth involvement and digital engagement, and impact on policy-decision-making at both Health Board and government levels.

Membership of the Action Group included 20 young people from two participatory youth initiatives focused on sexual health (Contraception Education Reform Team (CERT) & Waverley Care’s Young People Panel (WCYPP), sexual health improvement managers from three NHS Health Boards, a policymaker from the Scottish Government Sexual Health and Blood Borne Virus team, and a service development manager from a sexual health third sector organization. As planned, the Action Group met regularly online over a six-month period, with sustained engagement throughout the project duration. The group worked together on putting into action some of the key recommendations from the first phase of the CONUNDRUM research project. One of the main aims of the group was to show from a workable example the benefits of meaningfully involving young people in the sexual health policymaking process.

In evaluation activities, Action Group members have reported a variety of impacts derived from their participation in the Action Group, including development of new collaborative cross-sector relationships, strengthening of existing partnerships, development of new skills and knowledge relating to youth involvement and digital engagement, and impact on policy-decision-making at both Health Board and government levels.

Title SEED Intervention: a Stratified, Cluster Randomised Trial of a Multi-component Primary School Intervention that follows the Pupils' Transition into Secondary School 
Description The SEED study is a cluster randomised trial of a school-based public health intervention. We ticked the 'clinical trial' box because there was no option to tick for a cluster randomised trial of a public health intervention. The intervention is as follows: Data collected on emotional wellbeing of students is fed back to schools alongside benchmarked data and used to support reflective discussion sessions led by educational psychologists. This enables schools to make action plans at classroom level but also whole school level (interventions such as restorative justice). The focus of these interventions is particularly on managing transition from primary to secondary school. Trial results are currently being analysed. The report will be submitted mid April 2020. Scottish Government are interested in scaled up roll out. SEED has been developed and implemented in close partnership with Scottish government (for instance the Scottish Government is adopting questions from the SEED survey into the census to enable sharing across data sources/support consistency in what is collected) The intervention scoping, development and feasibility trial was funded by Scottish Government/CSO. Title STASH: Digital resources for young people's Sexual Health designed for peer-to-peer sharing via social media 
Description The STASH website houses a set of curated sexual health resources designed to be shared between peers via social media. The original STASH website was designed for the NIHR funded STASH feasibility trial [Moore 14/182/14]. Since the end of the Trial, we have used funding from the Scottish Government to implement key changes based on the findings of the feasibility trial. Specifically, these were: flexibility to share on any social media platform (previous site was linked to Facebook), enhanced search function and organisation of resources to make them easier to find, an escape button, simplified log in; improved design for both mobile and computer.

Results and Impact Malachi Willis led the Unit's Sexy Science engagement activity at this public engagement event. As people filed in for the event, they stopped by the table to guess when various sex-related events happened in history. Most figured that it was medieval times when the only church-sanctioned sex position was missionary, and plenty knew that the first Gay Pride parade happened in the 1970s. However, people were surprised that a husband could legally rape his wife until 1989 and that corn flakes were invented in the late 1800s to reduce masturbation! In honour of International Women's Day, the theme of the Smut Slam was taking control of your sexuality. Over the course of the evening, people of various gender identities got on stage to tell a 5-minute real-life story about their sex lives

It also offers some practical tips for anyone considering running online engagement workshops with young people. The blog was published to coincide with Scottish Co-production week. The organisers of the Scottish Co-production week shared the blog post on their social media as part of their calendar of events. Results and Impact As part of the CONUNDRUM Action project, we conducted eight online workshops to understand young people's priorities for digital sexual health information and support in Scotland. Workshop participants (n=35) were recruited via youth organisations, and were diverse in gender, sexuality, ethnicity, SIMD. Workshops were co-facilitated by trained peer researchers (n=4 young people) from the CONUNDRUM Action Group, alongside two members of the research team (Billie, Ruth or Carolyn). Peer researchers participated in three capacity-building sessions to strengthen their knowledge, skills and confidence relating to workshop facilitation, research ethics, and data management. They also co-designed the workshop with the research team. This piece of engagement work was prompted by requests from NHS health improvement staff and is intended to inform improvement of online sexual health resources. Based on our sharing of the interim report with our NHS partners, NHS Health Improvement staff reported a change in practice on the basis of insights from this report (ie deciding not to go ahead in the short term with a planned course of action relating to digital resources as it was felt these no longer met young people's needs, and instead further work was needed). A short final report on this engagement work is currently being co-developed with the peer researchers.

Results and Impact Between November 2019 and January 2020, we held 6 youth engagement events with young people aged 16-24 (n=38) in three NHS health board areas (Greater Glasgow & Clyde, Lothian and Lanarkshire), to inform the development of the CONUNDRUM study. We used dynamic activities (e.g. 'rich pictures', small group and whole group discussion) to explore young people's own perspectives on factors shaping condom and contraception use. Young people also advised us on the content and design (e.g. format, recruitment) of an online survey, which will be open to young people across Scotland in 2020.

Results and Impact We conducted three online workshops to evaluate impacts of the inter-linked CONUNDRUM and CONUNDRUM Action projects. We used Ripple Effects Mapping â€" a participatory evaluation method â€" to identify stakeholders’ views on key actions and impacts across both phases of the project. Workshop participants included young people, alongside professionals from NHS, academia, and Scottish Government. Next steps include working with stakeholders to analyse the map outputs and co-development of a journal article outlining impacts of a research-led multi-sector sexual health collaboration.

Results and Impact Met with a group of young carers from Swansea YMCA to showcase the work we do here in DECIPHer and ALPHA. this was an opportunity to recruit a wider range of young people to the ALPHA group. We ran a number of tasks looking at public health topics and the importance of asking young people for their views on topics. Another task we ran with the group was looking at the TRIUMPH network and discussing how to make our website more young person friendly and interactive. The group provided feedback on the most suitable and sustainable ways in which this website and TRIUMPH could appeal to more young people. including incentives, activities and usability.

This lead to an Editorial and longer article in the Dec 3rd edition of the Economist. The article was titled 'Talking about it at last' and the editorial was titled 'The wounds of silence'. As per Economist style I was not acknowledged in the article but I recognised many of the points as ones that I had made in our previous discussion.

Results and Impact We were invited by the Ayrshire and Arran NHS Health Board to give a talk on the CONUNDRUM study. The event was advertised widely in the health board - to anyone who works or could have contact with YP in acute and community settings. The aim was to present study findings and recommendations, and discuss these in more detail in relation to their specific context. The session was also recorded, so that it could be shared with those not able to attend. Attendees were very interested in study findings and mentioned how they had already changed some of their processes at a sexual health clinic based on the study findings. They were also keen to get involved in future CONUNDRUM related activities. The presentation was done by Carolyn Blake in collaboration with one of the study commissioners from NHS Lanarkshire ( Jo Duffy).

Results and Impact The paper, (Sweeting et al, Sexual harassment in secondary school: Prevalence and ambiguities. A mixed methods study in Scottish schools) was published in February 2022 - presenting findings from our Equally Safe at School project (whole-school approach to gender based violence we are working on with Rape Crisis Scotland). A press release designed with the journal (Plos ONE) led to interviews with several journalists (Sunday Times, Daily Mail, Independent) and coverage in a wide range of national and Scottish media. The story was covered on 'The Nine' (BBC Scotland News programme) including interview with our project partner at Rape Crisis. KM was interviewed on BBC Scotland. The research findings were put to Nicola Sturgeon at First Ministers Question time and she responded by referring to Government commitment to projects such as Equally Safe at School.

Results and Impact Mark McCann and Dominika Kwasnicka (Melbourne University) presented on the findings of the study to the Scottish Government alcohol harm prevention team, and Public Health Scotland staff.
There was a discussion about how the findings relate to other studies in the MUP evaluation portfolio and how the results will feed into the overall reporting on the evaluation of MUP for consideration by the Scottish Parliament. We also made plans made to co-ordinate press releases and online statements give a response to the study findings at the time of their publication.

Results and Impact Approx. 145 people attended our CONUNDRUM project Webinar in 2021 where we shared the key findings from our study, as well as the recommendations. The panel was chaired by Nicky Coia (NHS Greater Glasgow and Clyde) and the panel was made from research commissioners (NHS Lothian and NHS Lanarkshire) as well as a youth think-thank and charity working with young people. A 45 min discussion session followed presentations, where the overall feedback was very positive and engaging.
After the webinar, the research commissioners form the Scottish Government and NHS Health Boards mentioned they wanted to pursue a next research collaboration, following the findings from CONUNDRUM.

Results and Impact We published a research briefing summarising key findings and policy implications from one of our Natsal papers published in BMJ Sexually Transmitted Infections. We designed the briefing for a health practitioner audience (e.g. sexual health promotion workers, GPs, relationship counsellors and therapists). The briefing was disseminated via the Natsal Study and SPHSU twitter accounts, with the initial tweet from Natsal receiving over 3600 impressions and 274 engagements, including being retweeted by national sexual health networks and organisations, including British Association for Sexual Health and HIV and HIV Prevention England. The briefing was subsequently included in the Faculty of Sexual and Reproductive Health's e-bulletin on SRH and HIV policy.

Results and Impact The Sexuality and Abortion Stigma Study (SASS) team ran an interationally-attended webinar on Wednesday 3rd March 2021 to mark the launch of their project website and research briefings.
The SASS project re-examines qualitative data from a decade of studies on abortion across the UK. Drawing on the accounts of women who have sought abortion, and health professionals involved in provision, the project addresses abortion stigma, and considers how to move the conversation on from stigma to normalisation, both in academic and in more everyday terms.
In the run up to International Women's Day 2021, this webinar showcased the SASS study's findings, considered in the current context of Covid-19 and ongoing shifts in abortion provision in the UK.
The webinar was chaired by SASS Advisory Group member, and President of IPAS, Dr Anu Kumar. There were five short talks from the project team plus discussion.

Results and Impact Together with Shreya Sonthalia, I gave a two-day workshop on complexity in social science and public health as part of the SGSSS student-led training fund scheme. We gave introductory lessons on complexity science, agent-based models, and system dynamics using examples from the social sciences as well as public health and included exercises. Moreover, we taught the basics of both agent-based models and system dynamics in software programs typically used for academic purposes. The goal was to offer postgraduate students to learn about the opportunities of using the respective methods and give an introduction to each application. Several participants seemed inspired by the workshop and may use it in their future research.

For half a day we gave a brief introduction to complexity, discussed opportunities of using each method in social science and public health research and let participants engage in group activities. Results and Impact TRIUMPH Fest was a 2-day event to celebrate young people's contributions to mental health research and activism. The event attracted over 150 attendees including young people, researchers, policymakers and practitioners from across the UK. The vent included presentations from young people working on mental health projects, exhibitor stalls, panel discussion around working with young people and a series of workshops aimed at young people. Key outcomes included raising awareness of the different roles young people can play in mental health research, the identifying priority areas to improve young people's mental health, and developing new contacts and collaborations for future work.

Results and Impact In this Co-video Network Director, Jo Inchley, and Young People's Engagement Officer, Christina McMellon, talk about the work of the TRIUMPH Network in response to the Covid-19 pandemic. This video includes content on how the pandemic affected young people and the Network's work on the protecting the rights of the child during the pandemic. The video was recorded and published in collaboration with the UKRI Mental Health Research Matters team, and has been widely disseminated through both the TRIUMPH Network and the wider UKRI mental health networks.

Results and Impact As part of our ongoing coproduction work with young people through the TRIUMPH Youth Advisory Group - a group of 16 young people age 16-24 from across the UK - members of the group attended a 3-day residential weekend in Glasgow. The weekend included a several different workshops covering: analysis of research data from a Youth Advisory Group research project; development of new research ideas; reflecting on coproduction experiences and definitions; planning for future TRIUMH Network events.

Results and Impact The August residential brought together 16 young people, youth workers and triumph staff from across the UK who make up the TRIUMPH Youth Advisory Group. The 3-day residential included range of workshops focussed on delivery of ongoing TRIUMPH research activities and planning for future network activities. In particular, workshops included several analysing data and planning the next steps for the Youth Advisory Group's peer led research project "Felling Understood", a workshop focussed on planning for the TRIUMPH Fest event in October, and workshops reflecting on what teh Youth Advisory have achieved and what the groups' legacy is.

Results and Impact I taught on the Scottish Graduate School of Social Science 1 day workshop on participatory approaches to health and social research in UK and globally. In total, about 40 PHD students took part. It was a one day interactive workshop to provide training in participatory research methods to students from across Scotland who are considering/already using input from key stakeholders to maximise the impact and applicability of their research, including its potential to inform policy and practice.

Results and Impact Kirstin Mitchell was invited to talk to Scottish Government (civil servants) on how COVID-19 affected social relationships. The presentation was based on a paper co-written by researchers in the Relationships Programme which discussed four key relational mechanisms: social networks, social support, interactional norms and intimacy and how these were disrupted by the pandemic. 240 people attended and there was an engaged discussion following the presentation. Kirstin was subsequently asked to give the presentation again to the Wellbeing team with Scottish Government. Participants commented how helpful the talk was in helping them think about the impact of COVID on different aspects of social relationships. The paper was subsequently cited in a Scottish Government Pandemic Impact of COVID Evidence report

Results and Impact We ran two online validation workshops as part of the systems mapping process with about 25 public health and sexual health stakeholders and young people from our project Action Group. The aims of these 2 online workshops were to ensure the system map reflected the views of a broad range of stakeholders. It is still too soon to reflect on the wider impact of the mapping process. Publication of a paper and production of an online animation summarizing key messages will come out from April 2023.

Results and Impact The Natsal COVID study team, co-led by University of Glasgow, presented preliminary findings from wave one of their national survey on the impact of COVID on sexual and reproductive health in Britain. Over 500 sexual and reproductive health professionals registered for the event, including from across Europe and the US. Around 300 people attended live. The presentations highlighted strong age-related inequalities, with adverse outcomes more commonly experienced by younger people. The findings also underlined that although SRH services moved rapidly to reconfigure services, there were delays to access and unmet need affecting contraceptive use, cervical screening and STI diagnosis. A recording of the webinar is housed on the Natsal website and has been accessed by journalists (Women's Hour; Sunday Times) interested in writing up findings, as well as health providers and academics.
