Abstract

We know that your health is affected by the environment you live in, both positively and negatively. The local, national and international governments and organisations who influence and control these environments really want to design and manage them so that they help everyone stay as healthy as possible. Surprisingly, there is little evidence about what to do, how long it might take to have effects, and how big those effects on health might be. Our research tries to help. We know that the place you live is made up of many interconnecting things including natural and man-made environments, people, economy, and culture. These all work together to influence health. Our research is focused on understanding where, when and how places change, because then we can study the effects of those changes on health. We also study how people use their towns and cities because we can’t assume everyone stays in their neighbourhood all the time. We need to understand how and why they move around, day to day, and how that affects their health too. Finally, we use new methods to test out our understanding of how places affect health and how we could improve their design and management. These methods include using computers to simulate what does, and might, go on in neighbourhoods and cities, and the likely impact on residents’ health. Such simulations are very new to health research and we want to know if and when they can be helpful.

Technical Summary

We know that our living environment affects our health. One implication is that environment could be a lever to improve population health and narrow inequalities, but there is currently little robust evidence about what the best interventions or investments for health would be, how long they might take to work, and the likely extent of benefits. Building on our past work, the programme aims to tackle that lack of evidence and the reasons for it, in the next five years and beyond. A distinguishing feature of the Places programme is its focus on connections between environment, people and health. As social as well as epidemiological scientists, we conceptualise and research places as social, economic and environmental systems which shape and are shaped by individuals. In practice, this means we focus on time as well as space; a longitudinal perspective is needed to see how people and places affect each other.
The programme’s long-term aim is to understand how urban environments do, or can be modified to, protect and improve population health and reduce health inequalities. The objectives are to i) lead a step-change in the availability and application of novel data and methods to improve understanding of how places affect health; and ii) utilise these new methods and data to identify and evaluate place-based interventions for improving population health and reducing health inequalities.
We will deliver these aims via 3 interconnected workstreams. Workstream 1 creates and uses longitudinal data to tell us how places change over time and the consequences for population health and related behaviour. Workstream 2 collects and uses GPS data and approaches developed from ecology to tell us how people use places; how the content and configuration of settlements affects where they go, what they do, and what this means for their health. Workstream 3 creates dynamic simulation models to explore both how people, place and health are linked in systems, and how places can be changed to improve health and reduce inequalities. The health and health-related outcomes of primary interest are mental health and wellbeing, exposure to alcohol and tobacco environments, contact with nature, physical activity, and population-level indicators of health such as mortality rate.

Title Changes in the frequency, experiences and barriers to green space use in the United Kingdom during the COVID-19 pandemic (November 2020 follow-up) 
Description The aim of the survey was to be able to explore how the COVID-19 pandemic and restrictions/'lockdown' were impacting the UK population's visits to green space in November 2020 compared to data collected during the initial 'stay at home' phase of restrictions in April/May 2020 (http://dx.doi.org/10.5525/gla.researchdata.1038). The survey was conducted using an online interview administered to members of the YouGov Plc UK panel of 800,000+ individuals who agreed to take part in surveys. The survey was completed between 25th - 26th November 2020, with a nationally representative sample of 2,246 respondents. In the survey, respondents were asked about their frequency of green space visits, as well as their experiences within green space and barriers to visiting green space during the current stage of the pandemic and restrictions. Questions on garden access, pet ownership, household size, urban/rural status and long-standing illnesses/health conditions were also included. Demographic data was provided by YouGov (including sex, age, geographic region, ethnicity, social grade, marital status, working status, the number of children in household, government region and social media usage). Impact he model is designed to test and appreciate the difference in infections and casualties with and without social distancing/lockdown measures and, more importantly, to test the effectiveness of infection mitigation strategies such as large scale testing and contact tracing apps. Specifically, the complex interaction between the availability of testing and different levels of app adoption can be usefully explored with this model. The model also shows that, when we assume that the viral transmission runs predominantly through one's social network, the dynamic of the infection is different from that emerging under the assumption of most SEIR models of an equal probability of everyone infecting everyone else.

Title Data and code for: Local infectious disease experience influences vaccine refusal rates: a natural experiment 
Description Vaccination has been critical to the decline in infectious disease prevalence in recent centuries. Nonetheless, vaccine refusal has increased in recent years, with complacency associated with reductions in disease prevalence highlighted as an important contributor. We exploit a natural experiment in Glasgow at the beginning of the 20th century to investigate whether prior local experience of an infectious disease matters for vaccination decisions. Our study is based on smallpox surveillance data and administrative records of parental refusal to vaccinate their infants. We analyse variation between administrative units of Glasgow in cases and deaths from smallpox during two epidemics over the period 1900-1904, and vaccine refusal following its legalisation in Scotland in 1907 after a long period of compulsory vaccination. We find that lower local disease incidence and mortality during the epidemics were associated with higher rates of subsequent vaccine refusal. This finding indicates that complacency influenced vaccination decisions in periods of higher infectious disease risk, responding to local prior experience of the relevant disease, and has not emerged solely in the context of the generally low levels of infectious disease risk of recent decades. These results suggest that vaccine delivery strategies may benefit from information on local variation in incidence.

Title SPACES - Growing Up in Scotland: Cohort 1, Sweep 8 Physical Activity Data, 2015-2016 
Description The Growing Up in Scotland (GUS) study is a large-scale longitudinal social survey which follows the lives of several groups of Scottish children from infancy through childhood and adolescence, and aims to provide important new information on children and their families in Scotland. The study forms a central part of the Scottish Government's strategy for the long-term monitoring and evaluation of its policies for children, with a specific focus on the early years. Unlike other similar cohort studies, this survey has a specifically Scottish focus. A key objective of GUS is to address a significant gap in the evidence base for early years policy monitoring and evaluation. The study seeks both to describe the characteristics, circumstances and experiences of children in their early years (and their parents) in Scotland and, through its longitudinal design, to generate a better understanding of how children's start in life can shape their longer term prospects and development. Since 2005, study design and data collection have been undertaken by ScotCen Social Research with collaborations with the Centre for Research on Families and Relationships, based at the University of Edinburgh and the MRC/CSO Social and Public Health Sciences Unit over certain periods of the project. The survey design consisted of recruiting an initial total of 8,000 parents in 2005, comprising two cohorts of children (5,000 from birth, 3,000 from age two years and ten months), and then interviewing parents annually until their child reached age five years ten months. Further fieldwork was undertaken with the birth cohort when the children were around eight, ten, twelve and fourteen years old. A boost sample of 500 children from predominantly high deprivation areas was added to the cohort as part of the age 12 fieldwork. Data is collected via an in-home, face-to-face interview with self-complete sections. Fieldwork for sweep 10 was disrupted due to the COVID pandemic. As a result, the final portion of the data was collected via web and telephone questionnaires. Further information about the survey may be found on the Growing Up in Scotland website. SN 9120 - Growing Up in Scotland: Cohort 1, Sweep 8 Physical Activity Data, 2015-2016 The Studying Physical Activity in Children's Environments across Scotland (SPACES) project aimed to investigate the ways in which the built, natural and social environment influences children's physical activity. The project employed an observational, cross-sectional design that sub-sampled from Birth Cohort 1 (BC1) of the GUS during the GUS Sweep 8 fieldwork. Children sub-sampled from GUS were invited to provide objectively measured physical activity data by wearing an accelerometer for eight days. This dataset provides a range of summary physical activity variables from this project. A total of 775 children provided valid data. As a sub sample of GUS BC1, the summary level physical activity data can be linked, where appropriate, to other GUS BC1 datasets held on UKDS at the individual level. The physical activity data were collected between May 2015 and May 2016 by the MRC/CSO Social and Public Health Sciences Unit (SPHSU), University of Glasgow.

The blog provided an overview of the current context in relation to COVID-19, related research and our novel approach on utilising a whole systems approach to building an evidence base in this area. The blog was posted on Friday 5th June and by Wednesday 10th June it had 220 unique page views. The blog was promoted through Twitter and retweeted by a diverse audience, including researchers, a senior Government Minister (Maree Todd - Minister for Children & Young People), policy, practitioners and the Third Sector. Comments on social media highlighted the novelty and importance of our research in this area.

Results and Impact Our inaugural steering group meeting brought together representatives from research, policy and practice in the field of greenspace, including colleagues from Public Health Scotland, Greenspace Scotland, Forest Research etc. The meeting aimed to: i) establish our steering group; ii) discuss the wider GroundsWell project and the role of the cross-cutting theme on health inequalities; and iii) provide feedback on a theoretical model exploring factors that drive low and non-use of urban green and blue spaces. The meeting sparked discussion on barriers and facilitators to greenspace for those, in particular, who don't use or have low-use of these spaces. We will continue collaborative discussions in this area to advise on wider applicability and ensure the relevancy of our work.

Results and Impact Laura Macdonald created a blog post including findings from an engagement activity that she led upon (in collaboration with Historic Environment Scotland (HES)) at the ESRC 2023 Festival of Social Sciences (4/11/23) held at the University of Glasgow. The blog includes information on how visiting heritage may be linked to improved mental health/wellbeing, and findings from a short questionnaire which activity participants answered. The blog also provides a link to published research on heritage and mental health led by Laura Macdonald. The blog post was shared on social media, with HES, and with activity participants.

Results and Impact Laura Macdonald, and Laura Diffey-Higgins from Historic Environment Scotland, developed a table-top, public engagement activity for the 2023 ESRC Festival of Social Sciences event, in the Byres Community Hub. The primary objective of the activity was to present a visual representation of mapped heritage sites and explore residents' awareness of local heritage. The map served as a conversation starter to discuss connections between visiting heritage and the enhancement of mental health and well-being.

Participants were asked to provide their postcodes for an interactive heritage map to discover sites, such as listed buildings, scheduled monuments and designed gardens and landscape, within a 20-minute walk of their home. They were also given the opportunity to learn about the positive impacts on mental health associated with having heritage sites locally and visiting and engaging with heritage. The activity garnered a positive response with members of the public displaying a keen interest in the heritage sites within their local neighbourhoods. Some participants were surprised by the abundance of local heritage sites, in particular listed buildings, while others had limited awareness of the heritage in their local area. Participants commented that they found the interactive map to be particularly informative and enlightening. They expressed a newfound enthusiasm to seek out and explore heritage in their local area. They discussed specific sites that they were fond of, with one participant acknowledging that visiting heritage brought them happiness. Many expressed a shift in their perspective, having not previously considered the mental health benefits of visiting heritage sites, and expressed interest in finding out further information about potential benefits.

Results and Impact Dr Jonathan Olsen, Dr Jess Hepburn and Gillian Bell created an activity for the 2022 Festival of Social Science for Young People. We created an interactive online tool where Young People searched for their home, just like Google Maps. It then showed them whether there is a food shop, park, school, cinema or library, within a 10 minute walk of home. Via the Schools Health and Wellbeing Improvement Research Network (SHINE) network, we recruited 20 schools to take part. We then asked young people to think about what they have available in their local area and then complete the Place Standard Tool for Children and Young People. The tool has been adapted and developed by the Scottish Government, Play Scotland and A Place in Childhood, in consultation with children and young people. An video explaining the tool was also created. A total of 355 young people completed the online survey.

Results and Impact Dr Jonathan Olsen and Gillian Bell were successful in hosting a booth at the COP26 GreenZone and showcased a video that described research conducted at SPHSU of how built environment features, including natural spaces, in cities are associated with improved health and reduced health inequality. This was through an application to Universities UK. The GreenZone was accessible to the public, third sector organisations, academics, policy professionals and schools. Individuals from each of these sectors visited the stall.

Results and Impact Dr Jonathan Olsen, Prof Rich Mitchell and Dr Jonny Stokes participated in an evening network reception hosted at Scotland House, London, for politicians, third sector organisations, research funders and research organisations entitled 'Creating Healthier Places: A Place-Based Approach to Research & Partnership'. The evening reception took place on Tuesday 27th February 2024 and was attended by politicians, third sector organisations, research funders and research organisations from across the UK. The Places and Health team disseminated their research about 20-minute neighbourhoods and aspects of place that can benefit or harm health and health inequalities. The team created a 4-page evidence summary document which was provided to attendees. Prof Iain McInnes gave a keynote presentation that highlighted and discussed the Places Team research.

Results and Impact Dr Jonathan Olsen developed and led an activity at the Community Futures Day, as part of the Glasgow Science Festival 2023. The activity used an interactive place-making tool based on the Place Standard. We created a new interactive activity for the event where people decide what is good, ok and not so good about their neighbourhood, across categories including transport, access to nature and employment opportunities. They could also look up their postcode on the Scottish Living Locally Data Portal to see how connected their home is to 12 living locally domains. Around 200 people visited during the afternoon, including Patrick Grady (MP for Glasgow North) and Councillor Kenny McLean who stopped to chat about 20 minute neighbourhoods.

Results and Impact HealthyVille is an on-line activity for older primary aged children (aged 8-11 years) in which they can view a virtual "healthy" town, pick the health promoting features, and then read about associations between the built environment and health. The activity was promoted on twitter, Facebook, the Actify hub (https://www.actify.org.uk/module/887), and will be part of the 2020 Midlothian Science Festival Online Schools Programme (Oct 2020) and the Glasgow Science Festival (Sept 2020). We received feedback during the piloting of the activity that the children felt they gained knowledge of the links between the environment and health.

Results and Impact I presented at the Cross Party Group (CPG) on Health Inequalities on the theme of Active approaches to health inequalities affecting children and young people. My presentation focused on the results of the Active Healthy Kids Scotland Report Card, which was published in November 2021. After the presentation, there was a Q and A in which attendees were able to pose questions to the presenters. The session was chaired by MSP, Brian Whittle and attended by over 60 individuals from the private, public and third sectors. The Chair mentioned a desire for a parliamentary debate on the topic in the future and one Third Sector organisation has reached out to discuss collaborating on future activities.

Results and Impact Laura Macdonald undertook a lecture to post-graduate students on the Masters in Public Health course on 'the environment and inequalities in health and well-being'. The lecture provided students with information on health inequalities more generally and also allowed Laura to showcase our programme's recent research on inequalities in the built environment (i.e. work on walkability, environmental bads etc). Results and Impact This article explores the use of simulation techniques to measure population exposure to high levels of PM2.5 (particulate matter) in London. The author discusses the limitations of current approaches, such as mapping pollution fields based on average PM2.5 levels at home addresses, which do not account for individuals' actual movements throughout the day. To overcome these limitations, the author proposes using agent-based modelling (ABM), which creates a virtual world of London with individuals moving around the city and being exposed to PM2.5.

Results and Impact The PHASE Network funded a team of five researchers with expertise in modelling and public health to attend an NHS HackDay in Cardiff in May 2023. Hack Days provide an opportunity to bring together a wide range of skills in the same room and brainstorm collaboratively and come up with quick solutions to NHS challenges. The team from PHASE met NHS practitioners and people from a range of different sectors and disciplines at the event, and used this as an opportunity to promote the potential applications of modelling to health challenges.

In relation to this, they have posted our Nature-based early learning and childcare systematic review report for the Scottish Government on their resources page which enables those from Canada to access this report more easily.

After just over a week of being on the Outdoor Play Canada Website, the resource has had 47 views from 6 different countries: Canada (20), UK (14), Ireland (6), USA (3), Australia (2), and Germany (2) which demonstrates it's international reach.

Results and Impact This in-person Network meeting provided an opportunity to bring together modellers and public health experts from across the UK to meet others working in this field, and engage in discussion around methodological challenges, engaging decision-makers and effective use of evidence from agent-based model within public health. The meeting also included presentations from six research teams funded through the network, who have developed models across a range of public health areas including food advertising and the food environment, tobacco control, mental health, urban development and cardiovascular disease. Meeting attendees welcomed the opportunity to meet others working in the field, and the meeting generated plans for taking forwards further agent-based modelling work in public health.

Results and Impact The Places and Children's Health participatory workshop took place on Wednesday, May 3rd 2023, with an attendance of over 70 multisectoral stakeholders from various fields such as policy, planning, research and practice. The workshop explored the role of place in improving health and reducing childhood (birth to 18 years) health inequalities and provided a platform for engaging and collaborative discussions among key leaders in the field, passionate about leveraging the power of place to improve the well-being of children. The workshop started with four presentations from experts. They shared current Scottish policy and research related to places and children's health and discussed the importance of multidisciplinary and sectoral-wide collaborations. We then moved into four themed breakout rooms that reflect our research priorities: physical activity, active travel and play; mental health; 20-minute neighbourhoods; and unhealthy commodities. In each, attendees worked collaboratively to identify and prioritise research and policy questions related to the role of place on children's health. Findings from the breakout rooms were synthesised to inform a co-produced research-policy agenda, a summary of the most pressing research ideas. Since the publication of the report at the end of September, the blog which launched the report has had 402 views and the report itself has been accessed 88 times. Furthermore, three non-academic organisations have been in contact to explore potential collaborations which have resulted in productive discussions. The co-produced research and policy agenda has also been used as the foundation of a large research grant to the ESRC where we have highlighted that our proposed project has been informed by the pressing public health needs stakeholders identified during the workshop and as reported in the agenda.

Results and Impact Laura Macdonald undertook a presentation to Historic Environment Scotland (HES), the lead public body established to investigate, care for and promote Scotland's historic environment. The audiences included from those in relevant roles shaping approaches to heritage management, such as HES' Heritage directorate (i.e., those dealing with regulatory and heritage management policy work), and from Cultural Assets (i.e., those dealing with strategy, policy, and research around our properties in care). Audience members included Head of Designations, Deputy Head of Heritage Policy, Deputy Head of World Heritage and Head of Cultural Resources. Laura spoke about her research collaboration with Historic England (research/paper on heritage and mental health), and current collaboration with HES on children's contact with heritage. She also shared a heritage interactive map created for a Social Sciences festival. Audience members were interested and engaged during a short Q and A session after the presentation.

Results and Impact Working with the SPHSU Engagement and Communications team, we developed a bespoke SPACES webpage and created four infographics. The purpose of these was to showcase the SPACES project to a general audience, present key findings and highlight the impact it has had to date.

Since the new SPACES webpage was launched on 10th February it has had 205 hits, with 164 of those being unique page views. In addition, the infographics have been looked at approximately 35 times.

Results and Impact Working with the SPHSU Engagement and Communications team, we created a short video where Paul McCrorie briefly described SPACES, its impact to date, and the next steps. This video was posted on YouTube, Twitter (cut to 3 separate videos) and added to our Webpage. This video aimed to showcase the SPACES project to a general audience and highlight the impact it has had to date.

Over a few weeks, the YouTube video had 34 views. The reach was more significant on Twitter with a total of 191 engagements (video 1 = 146; video 2 = 27; video 3 = 18). We have also been contacted by two people to understand more about the SPACES project and potentially collaborate.

Results and Impact Outdoor Play Canada is a growing network of leaders and organizations working together to galvanize an outdoor play movement across Canada. They have a website that shares a list of news and resources related to outdoor play. In 2022, they asked us to write a short blog summarising key findings from our systematic review on the role of nature-based early childhood education on children's physical, social, emotional and cognitive outcomes.

This website has international reach and our blog post was advertised through other social media networks such as Twitter.

Results and Impact Scotland's First Minister visited the Clarice Pears building to meet with UofG Principal, Professor Sir Anton Muscatelli, and School of Health and Wellbeing colleagues, and to find out more about the health inequalities research being carried out by the Unit and across the School. Humza Yousaf MSP said: "Tackling poverty and protecting people from harm is one of three critical and interdependent missions for the Scottish Government - alongside our focus on the economy and strengthening public services. Research projects like those being demonstrated by the University of Glasgow are absolutely vital to Scotland's economic and societal growth. The School of Health and Wellbeing's research showcases the innovative thinking taking place at the University of Glasgow, which is typical of Scotland's world-class higher education institutions. The new Clarice Pears building adds tremendous value to the education and research being carried out at the University, creating a space for interaction between the university and local communities to address social and health inequalities."

Results and Impact Dr Jonathan Olsen, Prof Rich Mitchell and Dr Jonny Stokes led a Westminster lead drop-in session for MPs entitled 'Creating Healthier Places: A Place-Based Approach to Research & Partnership'. The drop is session took place on Tuesday 27th February 2024 and was attended by approximately 15 MPs from across the UK. The Places and Health team disseminated their research about 20-minute neighbourhoods and aspects of place that can benefit or harm health and health inequalities. The team created a 4-page evidence summary document which was provided to MPs.

Results and Impact This 5-part webinar series showcased current research that uses agent-based modelling methods to provide new insights into tackling pressing health improvement challenges. Each webinar focussed on a different area of public health: tobacco control; urban development; employment and welfare; cardiovascular disease risk factors; and food behaviours. Each of the webinars was attended by between 20-60 people, including those from research, practice and policy. There were a range of questions from the audience within each webinar and we have since received requests for further information after the webinars.
