Abstract
The aim of the Centre for Improving Population Health through E-Health Research (CIPHER) is to carry out research that will improve the health of the nation. It will do this by developing new methods to anonymise, link and analyse healthcare and similar data, to answer a wide range of important research questions. We will often work with researchers who have established large cohort studies, helping them make their cohorts more powerful, longer lasting, and cheaper to operate. E-health research is a new field and so CIPHER will be focussing on training researchers in how to use these data and methods.
CIPHER has four core research programmes: The first focuses on developing robust new methods to allow health and other information to be anonymised and analysed. For example, developing ways in which researchers accessing data cannot possibly identify the individuals involved, thereby ensuring that privacy is protected. CIPHER's other three research programmes focus on the impact on people's lives of (1) injury and the built environment; (2) mental health and substance use; and (3) infection. CIPHER researchers will undertake a range of studies to provide better insight into the causes of disease and injury, the consequences of these conditions on individuals and the population, and evaluations of treatments and policies which are designed to improve health. Knowledge of the longer term individual and societal impact of many conditions and treatments is currently very limited, as long term studies are very expensive. CIPHER's work will enable the long term follow up of outcomes using anonymised data, answering these questions effectively but at far less cost. Within its research programmes CIPHER will address many scientific questions, including: How effective are fall-prevention initiatives for older people? What is the impact of giving Vitamin D supplements to people in care homes in reducing hip fractures? Can linked data be used to help Fire and Rescue services prevent more home fires? Have interventions to make homes warmer reduced the number of deaths in winter? How much does substance use in young adults contribute to mental health problems? Does opiate substitution therapy in prison reduce the risk of death following prison release? Can linked routine data be used to identify opportunities to intervene in pathways to harm for people who self-harm or die by suicide? How can information be better used to reduce antibiotic resistant infections? Are the different micro-organisms we carry in our bowels involved in the development of serious health conditions?
CIPHER will work with the NHS to develop better ways to measure patients' experiences and treatment outcomes. CIPHER will also develop methods to enable linkage of anonymised treatment data to important social outcomes such as staying in work, continuing in education, or being able to live independently at home. Such information will help the NHS design its services to meet a broader range of patients' needs. CIPHER's believes it is important to communicate effectively with patients, patient groups and the public at large about its research. A significant part of our work therefore will be setting up ways to aid this communication. We will have public representatives involved in our research throughout and they will help guide us and help us communicate effectively with wider groups.
CIPHER will be one of a small number of similar Centres that is being set up in the UK and we will be working together to train researchers in these new methods and making sure universities have suitable jobs for them. CIPHER aims to be an internationally recognised centre of expertise in the use of electronic health and associated records for research and, along with other network members, position the UK as the world leader in the field, improving the population's health, increasing UK research income, and improving the economy.

Technical Summary
The vision of the Centre for Improving Population Health through E-Health Research (CIPHER) is to maximise the utility of routine health-related data on the UK population to support research to improve the nation's health. Objectives are: to provide a focus for innovative research using electronic health records; undertake novel linkage and analyses of large datasets; and build multi-disciplinary research capacity. The Centre will deliver four research programmes in: methodological innovation; injury, the built environment and health; psychosocial determinants of health; and, population aspect of infection. Much of the methodological work will be on enabling data to be used whilst protecting and ensuring privacy. The development of an anonymised data linkage platform will enable previously infeasible, large scale observational and interventional studies to be undertaken. CIPHER will address many scientific questions, including those listed below. How can data linkage improve measurement of the national and global burden of injury? What is the population impact of fall-prevention initiatives, Vitamin D supplements and targeted fire prevention activities? Have energy efficiency interventions reduced excess winter mortality? What is the relationship between urban design, obesity and mental health? To what extent do care and repair services protect health and prolong independent living? Is cannabis, alcohol and tobacco use associated with increased risk of psychosis? What is the burden of mental disorder in the young adult population from substance use? Does opiate therapy in prison reduce risk of death in the period immediately following prison release? Can linked routine data be used to identify opportunities to intervene in pathways to harm for people who self-harm or die by suicide? How can linked data be used to evaluate interventions to reduce antibiotic resistant infections? Does carriage of bowel microflora influence development of bowel disorders?
Planned Impact
There are many different groups who will benefit from the research programmes outlined in this bid. The first group is the UK and global academic community. Creating novel methodologies and innovative e-health technology platforms will enable previously infeasible and unaffordable cutting-edge, large scale, observational and evaluative studies to be undertaken, resulting in a step change in research capability, capacity and efficiency. This is turn will result in substantially enhanced scientific understanding of disease aetiology, disease prevention and management. The scientific outputs of such research will be reflected in high impact scientific publications and the development of new research programmes by funding agencies.
Much of the research to be carried out by CIPHER is designed to be relevant to a very broad cross-cutting policy agenda and we would expect the results of the work to influence UK Government and international policies in several areas. Themes such as health and wellbeing, the 'Big Society' social mobility, housing, and healthy ageing are reflected in current UK policy documents and statements. Over time the focus may be on other areas but the breadth and scale of data linkage envisaged in the CIPHER application will be capable of dealing with new policy concerns. One of the difficulties policy makers face is great uncertainty about the impact of policies, particularly those which affect multiple Government departments or sectors. The research programmes outlined in this application demonstrate the potential to link data across multiple sectors to answer these difficult questions in a series of innovative demonstration studies. For example, anonymised linkage of health, benefits, housing and other datasets, together with the development of information platforms capable of evaluating natural experiments, policies, and interventions led by health, local government or the third sector, will provide new knowledge which will influence policy at multiple levels, including those who create, implement or evaluate policies. One would expect the results of such research to lead to increased effectiveness of public services, enhanced quality of life, and improved economic performance through better health and a variety of other mechanisms. Improved health results in reduced costs from disability associated benefits, delayed requirement for assisted living and care in an ageing population, increased employment, and improvements in the competitiveness of the UK economy. A considerable growth in e-health employment is envisaged in both academic and private sectors to take advantage of the increased availability of data and skills resulting from the e-health research initiative. This in turn should result in an increase in the UK capture of global research funds, greater inward investment and increased employment which will benefit the UK knowledge and general economy.
Research funding bodies and researchers are increasingly asked to demonstrate the social impact of their research. This has proven to be quite a challenge as many traditional research projects take time to impact as funding is generally over too short a period to capture much of the longer term benefits. It is likely that the financial and societal benefits from health research are considerably underestimated. Description | The DPUK Data Portal has been relaunched to include a seamless transition from the DPUK main welcome site into the research infrastructure. The Portal, hosted by Swansea University, now includes openly available metadata on the DPUK cohorts, as well as information on the various parts of the infrastructure, which include: Portal analysis environment powered by virtual desktop infrastructure, imaging platform powered by XNAT, genetic platform created by Digital Health Labs, data linkage possibilities using world-leading matching technology, and a new and easy-to-use application form for researchers to apply for access to data, based on categorised dementia-specific data ontologies. The Portal is now supporting data access applications with dedicated resource to manage this process, and data management based on consultation with other data consortia and the cohorts themselves is underway using novel data management infrastructure (research data appliance based at Swansea University), enabling governance to remain entirely with the data provider and data access permissions to be managed by a central facility. |

Impact | The relaunch of the Portal has seen a surge in scientific collaboration in DPUK - for the period November 2017-January 2018, there are 12 live studies on the Portal, all using multiple cohorts and all cross-institution, including internationally-led studies from Maastricht University and the University of Hong Kong. These studies have primarily phenotypic data as their main modality, however we are also seeing the combination of phenotypic, genotypic and imaging data in studies, thus showing the power of a central system for accessing and analysing data from previously disparate sources. Genetics and imaging work packages are also contributing to the studies being supported by DPUK, and the Portal is making data available to specialists in these areas in order for them to derive services for the epidemiological community to make use of data that was previously unobtainable, either due to access difficulties or specialist intervention being necessary. |

Title | SAIL Dementia eCohort (SAILDeC) |
Description | DPUK has funded the creation of the SAIL (Secure Anonymised Information Linkage) Databank Dementia eCohort, known as SAILDeC. A team from the University of Edinburgh, led by Dr Tim Wilkinson and Dr Christian Schnier, have developed a research cohort from routine healthcare records found in the SAIL Databank, a research databank containing routine healthcare and adminstrative records for the Welsh populace. The eCohort is derived from 1.2 million individuals who met the inclusion criteria, and of those, ~130,000 dementia cases throughout the data coverage have been identified. This represents the first UK research-ready disease cohort derived solely from routine healthcare records, which can be analysed in its own right or combined with other rich sources of cohort and routine data. It is thought that the technical components of creating the cohort, in terms of coding and initial analysis, could be the lead for future eCohort generation in other disease fields. These eCohorts could then be analysed within their specific area of science, or combined for a rich multi-morbidity database. |

Results and Impact | As part of our comprehensive approach to improving burn care and prevention in resource poor setting, Interburns will be developing a Burn Prevention & Implementation Toolkit. The purpose of this Toolkit is to provide an evidence-based resource on the different tools available for primary prevention of burn injuries at the community level, which can be adapted to the specific needs and context of a country, region or individual community. In order to ensure this Toolkit can be effective in a wide range of settings internationally, the development workshop will bring together a wide range of professionals with relevant expertise from across the world. Ronan invited to participate in the development workshop for this programme, which is hosted in Swansea over 2 days on 2nd & 3rd July 2018, by the Centre for Global Burn Injury Policy and Research (CGBIPR) at Swansea University in the UK. |
