Exploring factors affecting adherence to asthma treatment using contextual data

Abstract

Asthma inhalers and treatment devices are increasingly incorporating the capability to 'track' the way they are used as part of our 'digital ecosystem'. This tracking can include information about the number, size and frequency of doses per day, as well as data associated with the individual using the device, such as their location, health state or environmental conditions (e.g. air quality). The potential of this data for clinicians, clinical researchers and developers, patients and their families is large, but the management and use of this data presents ethical and data analytical challenges. This PhD could explore questions such as 'how should data be presented to encourage patients adhere to treatment regimes?, 'how can clinicians responsibly gain value from multiple data sets containing personal patient data?' or 'how can clinical development use inhaler use data to inform the development of future treatments and delivery devices?'. This project will build on previous work conducted in partnership with colleagues at Nottingham University Hospitals Trust, and work in partnership with Glaxo Smith Kline

Planned Impact

We intend the Horizon CDT to be the place where partners come to find their future employees and to engage with the opportunities and challenges of digital identity and personal data. The key beneficiaries of our research will be:

- Commercial private sector companies that will engage with our CDT students during their research and/or employ them after graduation. Our partners include companies developing digital identity technologies as well as user companies across a range of sectors (consumer goods, entertainment, transportation, energy and others).

- Public sector and third sector organisations that are concerned with the use of digital identities to support civil society including broadcasters, healthcare providers and campaign groups.

- The public whose personal data forms the focus of their research and who will ultimately use and come to depend upon digital identities.

- Research communities spanning computer science, engineering, psychology, sociology, business and humanities.

These will benefit in various ways.

- Commercial, public and third sector companies will benefit from being able to recruit from a pool of talented PhD graduates who bring an in-depth understanding of digital identity and a proven ability to work in interdisciplinary teams. They will also benefit from being able to participate in co-creation of PhD research to ensure focus on relevant challenges and be able to exploit results of this PhD research.

- The public will benefit through a greater understanding of the opportunities and challenges of digital identity.

- Research communities will benefit by opening up promising new interdisciplinary fields.

Our Impact activities will be driven by Professor Derek McAuley, the Director of Horizon, who has a track record of establishing industry labs, spinning our companies and who is currently acting CIO of the TSB funded Connected Digital Economy Hub. Key activities will be:

- All Horizon PhDs will be carried out in collaboration with an external partner who will be involved in drawing up the initial topic, recruiting students, shaping the PhD proposal, supervision, and hosting at least one internship

- We will continue to organise knowledge exchange events within Horizon that are open to our network of over 100 external partners, including our annual Horizon Research Conference.

- We will encourage the release of applications, open source software, and open datasets wherever collaboration agreements allow.

- We will actively encourage our students to spin-out new ventures, including providing seedcorn funding through Horizon.

- We will engage our students with our two partner catapults, the Connected Digital Economy Catapult and the Satellite Applications Catapult.

- We will actively encourage industry visits through guest lectures on our "Broadening Horizons" core taught programme.

- We will also encourage companies to define, steer and sponsor the first year interdisciplinary team projects.

- Our students will complete a module on Public and External Engagement and are encouraged to engage in public events and exhibitions.

- Horizon's journalist-in-residence will help expose students research to the wider world through regular blogposts, while the University's marketing and communications team will help them develop press releases.

- We will provide training in research publication as part of the Professional Skills module and mentor publications through the Practice Led Project and the annual writing retreat (where students present and critique draft papers).

These impact activities will be supported by a professional online presence with posters, demos and podcasts made available through our website and associated YouTube channel and twitter feed, and with individual PhD profiles being posted on our own site and on external networking portals such as LinkedIn and ResearchGate.

Description Patients' perceptions about their condition have a strong impact on not only adherence to medication, but also on how they view themselves in the light of their condition and research implies that Twitter is a particularly rich source of perceptions, as patients frequently use internet for information sharing and support. However, Twitter contains a lot of noise in the form of tweets that do not relate to perceptions, but are rather generated to advertise research and corporate news and this kind of information could 'pollute' perception analysis. This research examined methods that could be used to extract perception tweets, on the example of tweets related to asthma. First, the differences between perception and non-perception tweets were demonstrated, in terms of their linguistic features. Secondly, the research focused on filtering perceptions using the classification process. Results demonstrated that there is a significant difference between perceptions and non-perceptions: perception tweets are shorter, have less capital letters, less punctuation signs and less hashtags. These features also performed well in predicting perceptions. However, the bag of words approach had better results in distinguishing between perception and non-perception tweets and the best results were obtained using word-based frequency vectorization and by training a neural network based classifier. Future research could explore the synergy of these approaches.
Exploitation Route Once the paper: Perception detection using Twitter is available online, researchers who do text analysis will be able to examine the methodology suggested and potentially apply it in their own work to filter the noise from publicly available data. There is also a useful implication for the business sector as filtering customer perceptions from non-perception tweets can be a powerful tool for marketing and business in general.

Description It is now largely accepted that interactions on social media, especially Twitter, constitute a socio-technical assemblage that creates a new public sphere where key aspects of civil society are played out. Twitter, as a social media channel, can offer significant potential for exploring empirical work that begins to unpack new social relations, as it is possible to analyze language within tweets and extract groups of topics that are discussed relating to asthma. Twitter API was used to stream tweets that have keywords: 'asthma' and 'inhaler', in order to get the relevant data set for the analysis. Data collected is in the English language and the streaming took place until 500.000 tweets were gathered.

Results and Impact This study was conducted as a set of interviews about knowledge, experience, behaviours and feelings about asthma. There was be 14 interviews (5 with people who have severe asthma and 9 with people who have mild asthma). The study was described using a brief description, explaining that perceptions and behaviours related to asthma will be the main topic of discussing, as well as experiences and feelings about living with asthma. This description was advertised to Psychology students through the research participation scheme. In addition, some participants were recruited using the snowball technique that will start through personal contacts of the researcher, as well as the initial Facebook post that contained similar advertising as a previously mentioned ad for Psychology students. The age range of participant was 18+ and their names were changed into pseudonyms. Interviews will be semi-structured, open-ended and last between 45 and 90 minutes. Participants were debriefed and got an explanation about the research goals.

Results and Impact This post was aimed at increasing the visibility of the research I conducted, related to COVID-19. The results, explained in the post, were based on a anonymized survey conducted in June 2020. The aim of the research was to raised awareness about the fears related to COVID-19 and now how surveyed participants can be categorised in 3 groups based on how they deal with these fears. As a result, the infographic that was posted online was circulated around the network of researchers and professionals in the data science field. This lead to a series of conversations related to segmentation and data visualisation techniques and writing a set of instructions for University of Nottingham researchers who were interested to learn more about best practices of data visualisation.
