NAMRA - Network for Antimicrobial Resistance Action

Abstract

Microbes continually evolve antibiotic-resistant strains despite the best efforts of biomedical scientists to combat them. This is taking us towards a future where routine operations and infections become high-risk, and where we cannot produce sufficient food globally (70% of antibiotics in the USA are used in animals for food production).

A new strategy is needed to combat Antimicrobial Resistance (AMR). This network will take world-leading Engineering and Physical Science (EPS) researchers and introduce them into a new Network for Antimicrobial Resistance Action (NAMRA). In a series of structured events, they will share their expertise with clinicians from the NHS, with biomedical scientists, and researchers from Health and Life Sciences. These people can tell the EPS researchers about the AMR problems that need tackling, and how any solutions must be designed to work in a real-world environment for use by healthcare workers, farmers, industry and the workplace. To help in this, the Network also includes leading researchers from Social and Human Sciences who can explain how AMR solutions must fit in with human behaviour, with Geographers who are experts in how distribution of the waters supply, and how livestock practices, affect AMR; with experts in the Legal and Ethical issues in developing new solutions to AMR for use in the wider world; and with experts in Business who research how supply chain issues affect AMR.

The EPS researchers have developed many world leading technologies, from the award-winning StarStream cleaning product, to surfaces that keep clean by mimicking shark skin. Such technologies were developed for other sectors (defence, nuclear etc.) and it is vital that such expertise be translated into the fight against AMR. Within NAMRA, the inventors can access the experts who understand AMR, and access laboratories and clinics to test the step-changing solutions they collaboratively identify. In return, world-leading work by current AMR researchers can be enhanced through NAMRA contacts to:
-engineer solutions;
-shape them for ready adoption by healthcare workers and others;
-set out the behavioural, ethical and legal framework for their adoption; and
-develop the business solutions so that, rather than staying on the laboratory bench, step-changing technologies can be fashioned into products that are available across the UK, and beyond.

The project begins with a 'Start-up' conference for attendees to share expertise and identify possible collaborators. Break-out sessions facilitate collaborative bids for NAMRA to fund 3-6 month projects to explore new ideas. One year in, a 'Community expansion' conference reviews the success of the collaborations to date, plans new collaborations, and invites AMR workers from across the UK, and representatives from NHS, Gov and local Gov, to discuss progress.

We will hold monthly meetings on particular AMR topics for smaller sub-groups within NAMRA. We will develop a Cognitive Computing facility to identify the knowledge gaps and possible fruitful areas of collaboration, working alongside the Steering Committee which performs its own assessment. We will also work hard to ensure that, after the two years of funding for NAMRA expires, we can sustain the network. Measures to do this include offering support, training and guidance:
-to ensure that interdisciplinary researchers do not 'fall into the cracks' between disciplines when publishing or applying for grants;
-to team-build an exhibit for public display on AMR, covering such issues as handwashing, biofilms and the use of antibiotics;
-identify and apply for sources of funding to continue their collaborations (incl. peer-reviewing proposals);
-to communicate their work to the public, via websites, school visits, Science Fairs;
-to the next generation of leaders in AMR.

The project ends with a 'Way Ahead' conference to ensure the good work continues after this funding ceases.

Planned Impact

The impact of effectively addressing AMR will be the avoidance of catastrophes in healthcare and food production. Its importance is widely recognized: the World Economic Forum suggested that AMR be added to the global risk register, and the World Health Organization has highlighted the serious implications for global public health, and the Dept Heath/DEFRA, "UK Five Year Antimicrobial Resistance Strategy 2013 to 2018" (Crown Copyright 2013) states: "There are few public health issues of greater importance than antimicrobial resistance (AMR).... AMR cannot be eradicated but a multidisciplinary approach involving a wide range of partners will limit the risk of AMR and minimise its impact for health ".

This last point is crucial: involving Engineering and Physical Science (EPS) researchers is seen by Dept Health & DEFRA as key. The goal of NAMRA is to become the first port-of-call for UK Gov concerning AMR. This requires NAMRA to present a coordinated, comprehensive, expert and visionary view, and to have established excellence in rigour and communication. Ensuring this will be the priority role of the PI.

It will be helped by the membership of NAMRA members on key advisory bodies. NAMRA includes:

-NICE committee developing draft guidance on 'Antimicrobial resistance: changing risk-related behaviours'
-HTA funding committee 2014 themed call for trials of interventions to reduce AMR
-2014 Longitudinal Prize Panel for new clinical tests to reduce use of antibiotics
-3 members (incl. PI) of Working Group of the Advisory Committee on Dangerous Pathogens Transmissible Spongiform Encephalopathies Sub Group
-invitee, WHO/UNICEF Joint Monitoring Programme for Water Supply & Sanitation Task Force Meeting on Methods'
-Member of the Chatham House Working Group on Antibiotic Delinkage
-European Pressure Ulcer Advisory Panel, part of the working group that developed the 2014 International Clinical Guidelines.

Contact has already been made with of the UK Gov Chief Medical Officer/Chief Scientific Adviser (CMO/CSA) and they have visited the University to discuss CJD and Ebola.

NAMRA is well placed to provide advice to these bodies, and to access UK Gov priorities so that its work can be steered towards the needs of DEFRA, Dept. Health, and the Office of the CMO/CSA. Furthermore NAMRA membership includes excellent examples of taking fundamental discoveries through to impact. Taking just the PI's award-winning StarStream as an example, it:

-has already accrued excellent preliminary results in AMR (incl. biofilms), sufficient for manufacturers and industry users to conduct their own independent tests (www.ultrawave.co.uk/starstream-1000-times-more-effective-than-water.html)
-one potential pump priming project will be to test this in conjunction with surfaces that inhibit bacterial growth and resilient biofilm formation, leading to enhanced horizontal gene transfer of antibiotic resistance genes between species in intimate contact, with applications in hospitals and food processors
-in NAMRA we have members in the University Hospital (Sterile Service, urology etc.) and members who already act as advisors to food retailers, particularly in meat, fish, dairy and poultry, who can advertise the results of a successful pump-priming project, quantify the benefits of adopting technologies, and furthermore inform the committees listed above of actual trial adoption and proven benefits
-and these exciting developments can be communicated to the public via TV & radio (for which StarStream has pedigree: www.southampton.ac.uk/engineering/research/projects/starstream.page?#media), and via the specific Exhibit, Schools, Science Shows and Outreach activities described in the Case for Support.

This route to impact of just one potential 3-month pump priming activity illustrates what the final report might say of the ~20 we will actually fund.

Impact The workshop on 25th November 2016 supported the Food Standards Agency in framing AMR-related policy work - both in the UK and in providing input to the global food standard body, Codex . As part of the workshop, food retailers agreed to be part of the bid to the ESRC Theme 4 call 'Tackling antimicrobial resistance: behaviour within and beyond the healthcare setting' . Our research team were successfully funded - £200,000 and the research will take place from February 2017 to June 2018. This is a multi-disciplinary team involving Geography, Engineering, Management and Microbiology.

New collaborations were establised, including one which went on to submit a joint grant proposal which has been awarded funding.

Results and Impact Professor Tim Leighton and Dr Marcella Hernández García, in collaboration with the Winchester Science Centre, took the NAMRIP exhibit to the Cheltenham Science Festival (one of the Uk's leading science festivals) in June 2017. Awareness was raised of AMR and the actions people of all ages can take to help address the issue. The impact was so great it caused a stream of children throughout the day to sing "happy birthday" twice through as they washed their hands in the toilet sinks

Results and Impact Professor Timothy Leighton and Winchester Science Centre Director, Ben Ward, were part of a team invited to exhibit 'The most dangerous game in the world' at the NESTA Longitude Prize event in London on 16th November 2017 as part of Antibiotic Awareness Week. GP and presenter of BBC2's 'Trust Me I'm a Doctor', Dr Zoe Williams introduced a range of speakers at the event, including expert patients, fellow GPs and funding representatives, all with a story to tell which highlighted the need for new diagnostics to improve antibiotic use. On either side of the presentations, networking opportunities gave over 100 delegates at the event plenty of chance to try the NAMRIP exhibit. This opportunity arose following the NAMRIP summer conference and following this event in November, the MRC have invited NAMRIP to take the exhibit to the Science Museum's Superbugs exhibition on 25th April 2018.

Results and Impact NAMRIP member Professor Xunli Zhang received funding to attend this event, jointly organised by BBSRC/NERC and CONICET (the National Scientific and Technical Research Council of Argentina). It was held in Buenos Aires on 10-14 September 2018. The workshop was attended by around 45 academics from Argentina and the UK, as well as representatives from the funding agencies involved in the initiative. It provided opportunities for participants to (i) better understand key challenges around AMR in Argentinian agriculture and more widely, and its impact on the emergence and spread of resistance in the environment that are relevant to other low- and middle-income countries (LMICs), and (ii) consider the research landscape and relevant expertise available in the UK and Argentina.

Results and Impact Professor Tim Leighton and NAMRIP manager Yvonne Richardson were invited to Parliament on 13 February 2019 to an event hosted by Stephen Metcalfe MP, chair of the Parliamentary and Scientific Committee and member of House of Commons Science and Technology Committee. The panel discussion was a launch of the UK's new 20-year vision and 5-year National Action Plan (2019-2024) (the UK's previous vision, the five-year Antimicrobial Resistance Strategy ran until the end of 2018, and was in part responsible for funding the formation of NAMRIP) and the UKRI Cross-Council Initiative on AMR. The UKRI produced a booklet which featured NAMRIP as a case study for success of the UK's mission in combatting AMR.

Results and Impact The conference on 14th December was about Antimicrobial Resistance and Infection Prevention. The break-out rooms featured an exhibition of inventions to tackle this threat while in the main hall there were talks by scientists, engineers, clinicians, social scientists, geographers and lawyers as well as 2 panel sessions with 'questions & answers'. The opening address was given by the Chief Scientific Adviser to the Food Standards Agency, University of Southampton Professor Guy Poppy (Centre for Biological Sciences) and the conference ended with a keynote address by Dame Sally Davies, UK Government Chief Medical Officer on 'Anti-Microbial Resistance: A Global Problem'.

- Treating cattle feel without antibiotics by trimming and draining infected feet and fitting wood shoe (Emma Roe, Geography and Environment);
- Improvements in the control of tapeworm, cysticercosis and sleeping sickness through analysis of infectious disease data and the relationships between human and livestock health (Nicola Wardup, Geography and Environment);
- Sustainable solutions to help prevent the infection, decline and extinction of fish populations, and the development of antibiotic resistance in farmed fish (Paul Kemp, FEE);
- Biomicrofluidic systems to carry urine past blockages in the ureter to prevent infection (X. Zhang, FEE);
- Engineered honey to replace systemic antibiotics in fighting infections in patients who suffer with chronic sinusitis and patients who have had sinus surgery (Sylvia Pender and Rami Salib, Medicine);
- Training audiologists for infection prevention in clinical practice (Nicola Timoney and Sarah Flynn, University of Southampton Auditory Implant Service, FEE)
- Domestic flood gates designed by an undergraduate team could prevent infection during floods (Martin Browne, FEE).

Results and Impact On 9 November 2018, Dr Parwez Hossain gave a talk at an event chaired by Professor Salim Khakoo entitled "Clinicians meet Engineers" at the University Hospital Southampton. Dr Hossain's talk was on one of his research projects which involved working with engineers to develop a microchip that can help detect eye infections, part of which was initiated by NAMRIP pump priming funding. The event was organised under the auspices of the MRC Confidence in Concept grant, an annual rolling grant won in part to replace the NAMRIP pump priming projects originally funded by EPSRC.

The project will develop innovative diagnostics to curb antimicrobial resistance (AMR), one of the biggest threats to global public health. The DOSA programme - Diagnostics for One Health and User Driven Solutions for AMR - brings together nine leading academic institutions, five from India and four from the UK, including NAMRIP member Professor Xunli Zhang.

Results and Impact On 10th September 2018 Dr Emma Roe presented at the Economic and Social Research Council funded 'Fresh Perspectives: Social Science in Antimicrobial Resistance Symposium and Networking Event', held at the British Academy London. Dr Roe explained her innovative mapping microbes research (funded by this EPSRC grant) and the value of a creative microbial aesthetic to engage people in more effective hand-washing practices. She also encouraged people to watch the public health education film 'In our hands' on YouTube, which uses striking magical images produced in the course of the research. She discussed the research with two visitors from the Ministry of Public Health, Thailand who have been trying to study the movement of pathogens in a hospital ward setting.

Part of Robert Hull's popular Biofilms stand 'When Slime gets Serious', the exhibit generated lots of interest with great feedback from the attendees which included industry sponsors, local school teachers and academic colleagues. Results and Impact At the invitation of the Medical Research Council, NAMRIP's 'Most Dangerous Game In The World' was transported to the Science Museum in London for the evening (25 April 2018), joining the activities in the UK Research and Innovation Superhero zone. Lates events are adults-only, after-hours theme nights which take place in the museum every month. They generally tie in with one of the exhibits at the Museum, in this case the 'Superbugs: The fight for our lives' exhibit which is on display throughout 2018. The Superhero's activity zone, in which the NAMRIP arcade game was the centrepiece, attracted over 1000 visitors. Volunteers from the NAMRIP network spoke to the visitors about the issue of AMR and how NAMRIP members are tackling it.

Results and Impact On 16 February 2016 Prof Leighton gave a Public Engagement talk to Hampstead Science Society, which covered a range of scientific projects in which he had been engaged, before covering many of the projects from NAMRIP's work on vaccination, infection prevention, and the mitigation of AMR.
The Society was formed in 1899 under the name of the 'Hampstead Astronomical and General Scientific Society'. The audience were the public, raging from school pupils to pensioners. They numbered around 40. Questions were encouraged and occurred throughout the talk, so many that indeed instead of catching the London Underground to Waterloo, 2 members of the audience fetched their car to drive me to waterloo so that they could continue the conversation.

David answered listeners questions on fungal nail infections (and related nail problems) to publicise the CapToe Study David is leading. This study is trialling a new treatment (cold atmospheric gas plasma) for fungal nail infection which aims to be more effective than current treatments and as a result reduce the need for antibiotics.

Results and Impact Professor Tim Leighton was an invited instructor on the Acoustical Society of America 2018 Summer School in Minnesota in May 2018. Around 100 PhD students spent the weekend talking to the ten instructors, in between specialist talks from those instructors, who covered a range of acoustics. Prof Leighton discussed how acousticians can widen their field rather than specialise, explaining how acousticians have interacted with NAMRIP in tackling antimicrobial resistance and promoting infection prevention through collaboration with researchers and end-users from a wide range of other disciplines.

Exhibit displayed at the NAMRIP Summer Conference on 5th June 2017. Attended by 100 delegates including Lord O'Neill and Lord Selborne. Before his keynote talk Lord O'Neill played the various games and admired its hands-on way of conveying the practical messages about AMR and what individuals of all ages can do to contribute to the solution. He took photographs of the exhibit and mentioned in his speech that he would be telling Dame Sally Davies about it at a high level AMR meeting. Following the conference, the exhibit was booked to appear at the New Forest Show.

Jon was an early and enthusiastic guinea pig in testing out our prototype hands-on outreach display exhibit.

Results and Impact Dr Paul Hurley exhibited Wash 'n' Watch, a prototype developed with NAMRIP pump priming funds, at the Cheltenham Science Festival in June 2017. It is an animated automatic handwashing machine, developed with assistance from Wallgate Ltd, to encourage proper handwashing.

Results and Impact As part of the Eco Bathrooms project, Charlotte Veal and Paul Hurley created an interactive Bathroom Jungle-themed game, communicating some of their work around AMR in the domestic sphere. Aimed at Key Stage two children, the game premiered at Winchester Science Centre during the half term holidays, and engaged hundreds of families in activities and conversations around AMR. Children were taken on a journey through a series of fun challenges - Biofilm Connect Four, Loo Seat Sponge Throw, and Drug Resistance Duck Race - to communicate some of the mechanisms of AMR and to talk to families about their domestic cleaning practices. This is part of the project 'Fighting Superbugs on the Home Front: Becoming an Ecological-Citizen in Your Bathroom', along with Emma Roe, Bill Keevil, Ian Williams, and with Owain Jones from Bath Spa University, exploring how citizens can connect domestic practices to the global threat of AMR.
