Care for Late Stage Parkinsonism

Abstract
The aim of this project is to evaluate the needs and provision of care for patients in the late stages of Parkinsonism and their carers in several European countries, to compare the effectiveness of different health and social care systems, and to lay the foundation for improved outcomes in this population. We will undertake an in-depth assessment of patients and their care arrangements in a population recruited through networks in six European countries. The systems and procedures that are used in the provision of care will be reviewed through a systematic literature review, interviews and assessments of patients, carers and health care providers, and through a trial comparing assessment by a specialist with management suggestions, guidance and access to telephone advice to that of usual care.
Through interviews, questionnaire assessment and review of current health-care and social care arrangement, we will assess the needs, provision of care and use of health-care resources, and their impact on patient and carer outcomes in different countries. National and regional databases will also be interrogated to identify current practice and use of health-care resources and drug usage. A systematic literature review of the evidence for effective management strategies, analysis of the study data, and evaluation of change in outcomes following specialist review will provide the basis for recommendations in the management of late stage Parkinsonism. We will also evaluate potentially useful outcome measures for use in this patient group.

Planned Impact
The information on current strategies and pathways for care provision and on interventions that are associated with better outcome in this study, both from the intervention part and the cross-sectional and longitudinal analyses, will provide important information to researchers planning studies on improved care provision. The results will also form the basis for an application for a full RCT of successful interventions of this group. As added value, there will be increased knowledge of different approaches to healthcare in different systems and of difficulties encountered by health and social care providers across European countries, which will supply social and economic studies in different health care systems.
The results may also provide a model for the research on better management of other chronic neurological disorders and age-related disorders in various health-care systems.

Description | We have gained significant insight into the patient experience of receiving a diagnosis of Parkinson's disease, the subjective experience of which has implications from the early to the late stages of the disease. This includes information on the key challenges for patients as well as carers, the issues of greatest impact on quality of life and functioning, the particular needs of patients in nursing homes, the patients' and carers' views on health care provided and unmet needs, and aspects of palliative care. We have provided an in-depth account of patient experiences at the late stage of the disease in the UK through qualitative interviews and quantitative assessments. The findings has been communicated internationally and nationally at conferences and through publications, and at stakeholder events at the European Parliament in Brussels. The results of the current grant has been feeding significantly into the content of the new grant on palliative intervention in this stage of the disease, and the development of a personalised tool to support self-management. |
Exploitation Route | These findings provide important information for improving care for patients at the late stage of PD taking into account their priorities, experiences and unmet needs. Ongoing grants aim to provide solutions for the issues identified, including grants from the European Commission, the NIHR, and the Royal College of Physicians. |

Description | The projected has yielded a number of publications providing information on the presentation, impact and health-care provision of late stage Parkinson's. As a consequence of the new information, there have been stakeholder events at the European Parliament, supported by the European Parkinson's Disease Association, two new project grants have been funded (one by the NIHR with the CLaSP PI as co-lead and one by the European Commission on Palliative Care in PD, in which the CaSP PI is a co-applicant). The results have been presented, e.g. at the annual Movement Disorders Conferences and at the Parkinson's Disease UK Executive Network in London, meetings on Parkinson's disease and Related Disorders and a conference on Non-motor symptoms in Parkinson's. Overall, the challenges of the late stage of Parkinsonism, on which there was previously little information have become much more understood, and our results have contributed to the growing knowledge on this difficult to manage, disabling disease stage. |

In order to maximise the information gained from both studies and to complement the research from both projects, AS have been acting as the scientific advisor to the EPDA project and facilitate information exchange. I have advised on the methodology and content of a European-wide survey in people with PD, carers and health care professionals, and presented the results to members of the European Parliament in Brussels. |

The aim of the symposium was the exchange of information, challenges, results, and further discussions across the different areas of research, led by leaders in their respective, diverse fields in neurodegeneration. A vivid interchange, both during the workshops and in between stiumulated ensuing exchange of ideas and further collaboration. So far, this has resulted in the current organisation of a further meeting, and plans for the development of guidelines. |
