Abstract
Dementia is not a "developed world" condition: there are already more people with dementia in LAMICs than in high-income economies, yet LAMICs are typically less equipped to respond to the high and increasing prevalence. By 2050, there will be 90 million people with dementia in LAMICs (Prince et al 2015).
People with dementia, particularly at more severe stages, require intensive care and support, which is very costly. These costs are mostly borne by unpaid family carers, primarily women, who often have to leave paid work, risking personal impoverishment and societal productivity losses. LAMICs face rapid growth in numbers of people with dementia without well-developed or well-funded health and care systems. Family care availability is decreasing as a result of demographic, societal and economic changes.
We will build research capability using economics, epidemiology and policy analyses to help LAMICs respond to the needs of the growing numbers of people with dementia in an ethical and sustainable way. The co-applicants, from the UK and South Africa, have strong track-records in dementia research in high-income countries (HICs) and LAMICs, research on health and long-term (social) care in LAMICs. We will partner Alzheimer's Disease International (ADI), a federation of 85 national Alzheimer/dementia associations.
We will work with local researchers and NGOs in 7 LAMICs: Brazil, India, Indonesia, Kenya, Jamaica, Mexico, South Africa. To build capability we will offer formal training in research methods, application of those methods to generate new evidence and tools, and training and practice in use of evidence to inform policy. We will apply the best methodological approaches and publish our results in peer-reviewed journals, but more importantly we will ensure that our research generates practical tools for use directly by stakeholders to develop services and improve practice, or to influence policy.
We will use systematic reviewing and meta-analysis to review evidence on what works in LAMICs and what can be delivered in particular contexts, but also make that research available online and in DVDs as evidence summaries written in plain (dementia-friendly) language and translated to relevant local languages.
We will use Theory of Change (ToC) to co-develop with local partners (researchers/NGOs) and stakeholders (including people with dementia and carers) the research and training agenda for the project to ensure that our activities achieve maximum impact. ToC will also help us develop indicators to evaluate the project's impact.
Our research programme will involve development and evaluation of an intervention to increase dementia awareness and reduce stigma, a qualitative study of costs and other impacts of providing family care to people with dementia in different contexts, generation of quantitative evidence on impacts and costs of dementia on individuals and families, and instruments to collect these data.
We will use the evidence generated to develop, for each country, credible estimates of the costs and impacts of dementia, and use simulation modelling to project future dementia care needs, the health and social services required to meet those needs, their costs, and the impacts of implementing evidence-informed dementia care pathways and better coverage. Models for each country will be simple to update and adapt.
We will assess the policy implications of our projections, consider the barriers presented by current organisation and financing systems and the availability of trained workers, and outline reform opportunities to improve dementia, considering the wider health and social care systems.
Finally, we will work with ADI and other policy partners to produce local recommendations to support the implementation of National Dementia Plans. We will organise stakeholder workshops in each country to present project outcomes, and high level stakeholder meetings in each of the regions, in collaboration with WHO.

Planned Impact
Dementia is a relatively new, but fast-growing challenge for LAMICs. We will work with local researchers and NGOs in generating the research evidence and policy tools needed to support "evidence-based policy-making". This is why our overarching objective is "contributing to improve dementia care, treatment and support systems so that people living with dementia are able to live well, and so that their family and other carers do not shoulder excessive costs or risk impoverishment or experience their own health problems".
The team of academic applicants, based in the UK and South-Africa, and ADI and other international partners, will work with local researchers and NGOs in Brazil, India, Indonesia, Jamaica, Kenya, Mexico and South Africa. Through ADI, we have contacted local Alzheimer's or dementia associations in all of these countries, and all have expressed very strong interest in participating in the project. Each of them has also identified academic researchers working in the area of dementia. For example, Alzheimer's & Dementia Organisation Kenya (http://www.alzkenya.org/) responded: "We have been thinking of carrying out our own research in Kenya so we can understand the numbers affected by the disease and this angle of the project on how to 'respond to the challenge of meeting the needs of the growing numbers of people living with dementia in a sustainable way' is crucial to us as it helps us understand how to meet the needs of the people affected by the disease and the caregivers too."
Members of the team worked closely with local researchers and NGOs in Indonesia, Mexico and South Africa during the elaboration of the most recent World Alzheimer Report published by ADI (https://www.alz.co.uk/research/world-report-2016). That collaboration has had a very important influence in the gestation of this project, as it highlighted both the lack of information about the current situation of people with dementia and carers in most LAMICs, and the importance of having that knowledge in order to develop policies. The report also highlighted the lack of available evidence about what works in the care, treatment and support of people with dementia in LAMICs.
First, we will use Theory of Change (ToC) to co-develop the research and training priorities for the project overall and, working with local stakeholders, also in each country. The ToC process makes explicit the project's outcomes and identifies a hypothesized causal pathway for the programme to achieve its impact. This is used to identify indicators for all the outcomes, which will be used to monitor progress for each outcome in the short, medium and long-term, and finally to evaluate the project and each of the workpackages.
Following the priorities identified, we will offer formal training in research methods, application of those methods to generate new evidence and tools, and training and practice in use of evidence to inform policy. On completion, all the tools and models developed as part of this project will be made publicly available, building on the co-applicants' experience of, for example, the Dementia Evidence Toolkit developed in the UK as part of the ESRC/NIHR-funded MODEM project (http://toolkit.modem-dementia.org.uk/).
All project research and policy tools will be designed so that they are easy to update and adapt, and they can continue to be useful well after the end of the project, both by the local researchers and NGOs involved in this project, but also by other researchers, NGOs and policy-makers, both in the 7 study countries and more widely.

Description | The STRiDE Project team is working on papers and reports linked to research. Two journal special issues are being finalised to draw together key findings from across the project - one for the International Journal of Geriatric Psychiatry and one for Dementia. Wider outputs are ongoing. covering findings from the project: • Theory of Change workshops (WP1) were completed in all seven STRiDE countries, and reports from each country finalised. An overview paper has been submitted to a journal and is awaiting a decision. A research tool on how to conduct a STRiDE-style ToC workshop has been published. Each country's ToC map is being finalised for online publication. • Focus groups on stigma (WP3) were completed in most countries and key messages are being drawn out in journal papers; papers from Brazil, India and South Africa have been published. • A systematic review of studies estimating the prevalence of dementia (WP5) has been published in the journal Global Health Research. • The desk review and SWOT analysis parts of the in-depth situational analyses (WP7) were completed in almost all countries; findings from work on case vignettes, mapping care pathways and validation interviews with stakeholders (including experts by experience) are being finalised for submission to relevant journals. • A research tool on conducting literature reviews has been published, and further tools are in development. • STRiDE partners developed a number of reports on the impact of COVID-19 on people living with dementia within their countries which were published on the LTCCovid.org website curated by the STRiDE co-lead, Adelina Comas-Herrera. These included reports on the situation in each of the STRiDE countries as well as a report focusing on low- and middle-income countries more broadly. Partners presented at a number of events and workshops focusing on the impact of COVID-19, including through LTCCovid.org webinars and Corona Older events. |
Exploitation Route | Some of the outputs are in publication and communications around these are in development. We expect research evidence from WP3 (stigma) and WP7 (case vignettes, care pathways and situational analyses) to generate significant interest within each country, and discussions have already taken place with relevant stakeholders (for example, discussions with Ministries of Health in Indonesia and Kenya). Reports produced for LTCCovid have been of interest to local and national decisionmakers. These informed the World Health Organization's Policy Brief in July 2020 on preventing and managing COVID-19 across long-term care services which specific examples included from each report (https://www.who.int/publications/i/item/WHO-2019-nCoV-Policy_Brief-Long-term_Care-2020.1). |

Description | As the last 6-8 months of the project were heavily dominated by contingency planning as a result of the impacts of COVID in each country, and because we have spent the months since the end of the project completing some activities - such as analyses of data and writing up outputs - we cannot go into detail at this stage on how findings have been used. Some findings have been published, particularly from work on stigma, while other papers are submitted or being being finalised for journals, including two special issues of journals dedicated to work from the project. Nevertheless a number of local impacts are emerging, including contributions to local dementia plans. More detailed information on how we envisage the outcomes from the research to be taken forward will be provided, and on how it has been taken forward, in our next reporting period. The project benefited from partnership arrangements across seven core countries: Brazil, India, Indonesia, Jamaica, Kenya, Mexico and South Africa. All partners actively engaged stakeholders - a key priority for STRiDE - and built on earlier steps to engage with policy opportunities, for example in community outreach events, in meetings with policy-makers and through radio and TV interviews, and newspaper interviews and features. Particular efforts were made during World Alzheimer Month in September each year during 2018 through to 2023. The STRiDE project was also presented at a Parliamentary Committee meeting in Brazil in June 2019. In addition, STRiDE featured in ADI's World Alzheimer Report 2020, 2021, 2022, 2023 and will feature in 2024. Partners gave a number of presentations on STRiDE at local, national and international conferences and events, and multiple presentations at the ADI conference in 2023. Partners also participated in a number of activities linked to World Alzheimer's month through the NGO partners in each country. Country teams organised national launch events to publicise the project and engage stakeholders early, in addition to engagement through Theory of Chabge (ToC) workshops and national advisory groups, and have continued to use opportunities to publicise the project at events and meetings. Presentations have also been given, by invitation, at meetings of the OECD (May 2019), the World Health Organization (July 2019), the Inter-American Development Bank (November 2019) and the World Dementia Council (Tokyo, October 2019). As part of the full STRiDE meeting in Cape Town in March 2019, the South African team organized a national stakeholder event which included representatives from the Ministries of Health, Social Development and the South African Human Rights Commission, as well as people living with dementia and care providers. Steps are continuing to be taken to support impact from the STRiDE project. Opportunities to engage with the development of national dementia plans are being taken, for example in Kenya and India; a project team member (Wendy Weidner) took part in a World Health Organization regional meeting in the EMRO region in Beirut (Lebanon) to discuss supporting the development of national dementia plans, as well as a similar meeting in Brazzaville, Congo, focused on the Africa region; and colleagues are joining national groups to support their activities such as membership of the dementia care guidelines working group for Indonesia, and the Ministry of Health working group for dementia in Kenya. Country teams continue to use opportunities to engage with policy discussions that could be relevant for STRiDE, such as on policies for older people in Indonesia. A number of activities are underway to support wider engagement outside of STRiDE countries, as well as enhance the impact of STRiDE's activities. For example, we developed links with the Global Brain Health Initiative and have opened up STRiDE training opportunities to GBHI Fellows. The Asian development Bank has a programme, Strengthening Capacity for Developing Member Countries in Elderly Care, which is very relevant to the work of the STRiDE project. We are in discussions about the possibility of collaborating in a survey and joining forces in capacity building; we have drafted a Memorandum of Understanding to support the development of this collaboration. Adelina Comas-Herrera UK (co-lead) is a member of the advisory group of the Global Health Research Group on Dementia Prevention and Enhanced Care (DePEC), an NHIR-funded research initiative led by Newcastle University which involves researchers in Malaysia, Tanzania and Kerala. Funding has been sought to support STRiDE activities in other countries such as Hong Kong, Romania, Switzerland and England, and we are working with contacts to support STRiDE-based activities in other countries. Title | Dementia anti-stigma intervention: Facilitator's manual - Train-the-trainer program |
Description | This manual was developed within the Strengthening Responses to Dementia in Developing Countries (STRiDE) project using the Medical Research Council (MRC) framework for the development and evaluation of complex interventions. STRiDE was implemented in seven countries (Brazil, India, Indonesia, Jamaica, Kenya, Mexico and South Africa) between 2018 and 2022 and aimed at building local capacity to support the development, implementation and evaluation of national strategies to deliver appropriate, equitable, effective and affordable dementia care. One of the objectives of STRiDE Kenya team led by Professor David Ndetei, Dr. Christine Musyimi, Mrs Elizabeth Mutunga and Mr Levi Muyela was to reduce stigma by increasing understanding and awareness of dementia among healthcare providers and community members. A situational analysis to better understand baseline perceptions among: members of the general public, healthcare providers (clinicians and community health workers) and carers of people living with dementia was then conducted to respond to this need. This resulted in a dementia anti-stigma intervention that was tested in a rural setting in Kenya. The manual can also be used in similar low resource settings. |

PI Contribution | As part of the collaboration there are quarterly calls between the GBHI team and STRiDE (Martin Knapp and Adelina Comas-Herrera) to identify areas of overlapping interests and potential for specific collaborations between the GBHI fellows (and staff) and the STRiDE project. Results and Impact | The Regional Policy Dialogue is an Inter-American Development Bank initiative that seeks to facilitate dialogue between Ministers and Deputy Ministers to exchange experiences and ideas on policy opportunities that contribute to the development agendas of Latin American and Caribbean countries. This event featured the participation of worldwide experts and IDB specialists to learn about and debate four key issues in the design and implementation of a long-term care system: Who to provide long-term care services for? What services to provide? How to ensure the quality of long-term care? How to finance the system? In addition, the event offered the opportunity to discuss the role of technology n long-term care, among other topics. Adelina Comas-Herrera was invited to present on the implications of dementia for long-term care systems, introduced the STRiDE project's approach and, with Mariana Lopez-Ortega (from STRiDE Mexico) and Ricardo Pérez-Cuevas (a Senior Health Specialist from the IDB who is in the STRiDE Jamaica National Advisory Group held a panel session discussing experiences of carrying out STRiDE in Mexico and Jamaica and the value of this approach (this is available on video). Adelina's presentation also included material from the MODEM project. Following this presentation, Adelina Comas-Herrera has been in discussions about the possibilities to carrying out STRiDE in other countries in the American continent, including Argentina, Peru and Cuba. |

Results and Impact | This was a workshop organised by the World Health Organisation to "support the implementation of the WHO global action plan on the public health response to dementia 2017-2025". During this workshop I acted as an external expert, facilitating discussions of ways forward in dementia policy development with groups of policy-makers and NGO representatives from various countries. The discussion with the representative of the ministry of health of India was particularly encouraging as, by considering the option of adding dementia to existing National Plans on NCDs and mental health rather than having to develop a whole new dementia plan, he could see a potential avenue to contribute to the development of national policy in dementia that he had not considered before. India is one of the countries in the STRiDE project and we plan to continue that dialogue as part of the project's work. |

The Masterclass will also provide early-career researchers an opportunity to discuss practical and methodological issues in social and economic research in dementia with leading world experts in dementia. This Masterclass has generated at least five follow up discussions about potential collaborations, particularly with the Global Brain Health Institute. |

Results and Impact | A Theory of Change workshop was hosted at LSE on 9 and 10 March 2018 for the STRiDE project. This led to the development of a ToC map which will feed into the monitoring and evaluation process for the project. Participants were country researchers and Alzheimer's Associations in seven countries - Brazil, India, Indonesia, Jamaica, Kenya, Mexico and South Africa - who are all partners in the study, and the international team from LSE, Brighton and Sussex Medical Science, Alzheimer's Disease International. An individual living with dementia and her carer participated as representatives of Dementia Alliance International. Our ESRC liaison officer participated and a colleague from the World Health Organization. This involved discussion and questions on the project's potential impact, its activities and mechanisms to achieve impact. It also acted as a training event for the 7 countries who will carry out the same workshop nationally. |

Results and Impact | Wendy Weidner has been invited to be a member of the advisory committee of the CST-International project, led by University College London. As part of the group she has been able to provide advice to strengthen the project and consider links with the STRiDE project, as both projects are considering responses to dementia in developing countries. The CST-International project focuses on providing evidence-based, group non-pharmacological intervention which can significantly improve cognition and quality of life for people living dementia. It is low cost, does not require specialist equipment and can be provided by community workers. This project focuses on an upper middle (Brazil), a lower middle (India), and a low income (Tanzania) country. |
