COVID-19: Data and Connectivity – National Core Study (D&C-NCS)

Lead Research Organisation: Health Data Research UK (HDR UK)

Abstract

The COVID-19 Data and Connectivity National Core Study will build a national health data research capability to support priority COVID-19 research questions. It draws on lessons for health data research which have been learnt from COVID, showing how urgency and focus during the pandemic have enabled data to be used to generate insights that transformed our knowledge of COVID-19. These lessons highlighted significant challenges with data access, fragmentation, the importance of transparency and concerns over inherent inequalities and sustainability across approaches.
The COVID-19 Data and Connectivity National Core Study will therefore ensure needed datasets are discoverable and accessible and linkages are established to answer the research questions prioritised by the five National Core Studies. These research questions and the supporting data and connectivity infrastructure will be critical in provisioning for the longer-term response to COVID.
Delivery of the COVID-19 Data and Connectivity Study will involve close interaction with data custodians, the public and patients, and providers of UK-wide national Trusted Research Environments (TREs) to ensure the required data is stored safely and securely, made readily available to approved researchers and is associated with compute, analytical and data services that make it easier to address priority research questions in a transparent and trustworthy way.

Technical Summary

The Data and Connectivity study sits across the other National Core Studies and aims to build a national health data research capability to support COVID-19 research questions, ensuring datasets are discoverable and accessible and linkages are established to answer the priority research questions from the other five National Core Studies. Making data available for wider research use will increase the scope of benefits beyond the specific studies above, leading to unexpected benefits and boosting UK research capacity more generally, increasing return on investment for the NCS programme.
Data integration and harmonisation of methods and standards will enable rapid research and development of new interventions and technologies across the spectrum of COVID-19, and knowledge and technology transfer to other clinical and public health areas.
Over the next 6 months the Data and Connectivity study will:
1. Map the initial high priority COVID 19 datasets required by the National Core Studies.
2. Deliver the necessary data infrastructure and services (quality and timely data, ability to link the data and provide access to data for multiple researchers) in 5 Trusted Research Environments across the UK to allow the initial high priority research questions to be answered efficiently in a transparent and trustworthy way.
3. Deliver a single “shop window” for the COVID-19 National Core Studies to ensure the priority datasets for COVID 19 research are findable, accessible, interoperable and reusable (FAIR) by enhancing the UK Health Data Research Innovation Gateway
Collation and linkage between datasets is critical to bringing the core studies together, ensuring that each of them can deliver against their policy priorities e.g. hospital data may not currently be linked with GP data and wider community data (e.g. socioeconomic data or data on housing and the built environment). Access, cleaning, linkage and use of these datasets together is needed to fully understand links between these factors and outcomes.

Title GPES Data for Pandemic Planning and Research Dataset (COVID-19)
Description NHS Digital's fortnightly collection of GP data will provide data to support vital planning and research into coronavirus (COVID-19). Coronavirus (COVID-19) has led to increased demand on general practices, including an increasing number of requests to provide patient data to inform planning and support vital research on the cause, effects, treatments and outcomes for patients of the virus. To support the response to the coronavirus outbreak, NHS Digital has been legally directed to collect and analyse healthcare information about patients, including from their GP record, for the duration of the coronavirus emergency period, under the COVID-19 Public Health Directions 2020 (COVID-19 Direction). All GP practices in England are legally required to share data with NHS Digital for this purpose under the Health and Social Care Act 2012. More information about this requirement is contained in the Data Provision Notice issued by NHS Digital to GP practices.

Title Health Data Research Innovation Gateway
Description The Health Data Research Innovation Gateway (the 'Gateway') provides a common entry point to discover and enquire about access to National Core Study datasets prioritised by the Data and Connectivity NCS (in addition to other non NCS UK health datasets for research and innovation). It provides detailed information about the NCS datasets, which are held by D&C NCS Delivery Partners TREs such as description, size of the population and provides a harmonised data access request process for access. The NCS Reporting Dashboard tracks the numbers of data access requests for NCS data assets here https://www.healthdatagateway.org/pages/national-core-studies-ncs-dashboard, and the NCS Collections page contains details of all the NCS data assets , projects and publications arising from NCS studies: https://www.healthdatagateway.org/collectioncategories/national-core-studies The Gateway includes the ability to search for research projects, publications and health data tools, such as those related to COVID-19. New interactive features provide a community forum for researchers to collaborate and connect and the ability to add research projects. The Innovation Gateway does not hold or store any datasets or patient or health data but rather acts as a portal to allow discovery of datasets and to request access to them for health research. A dataset is a collection of related individual pieces of data but in the case of health data, identifiable information (e.g. name or NHS number) is removed and data is de-identified where possible. When you access the Gateway you will not be able to view or extract the data itself. Instead, you will be able to see information that describes what the different datasets are (e.g. where the dataset has come from, a description of the dataset, the time period and the geographical areas the dataset covers). In order to access the data, you will need to sign in and then follow the access request process. Only once your request has been through the process to ensure your application meets all of the requirements and is approved by the data controller will you be able to access the data. Your access to the data will be provided outside of the Gateway in line with the processes of the data controller - read through our FAQs for more information on the process. The datasets that are discoverable through the Innovation Gateway are from organisations in the NHS, research institutes and charities, which are part of the UK Health Data Research Alliance. You can find out more about its members on the UK Health Data Research Alliance website. The information provided about the datasets has been provided by the relevant data controller and is continually updated.

Title ONS 2011 Census Wales (CENW) Dataset
Description Every ten years since 1801 the nation has set aside one day for the census - a count of all people and households. It is the most complete source of information about the population that we have. The latest census was held on Sunday 27 March 2011. Every effort is made to include everyone, and that is why the census is so important. It is the only survey which provides a detailed picture of the entire population, and is unique because it covers everyone at the same time and asks the same core questions everywhere. This makes it easy to compare different parts of the country. The information the census provides allows central and local government, health authorities and many other organisations to target their resources more effectively and to plan housing, education, health and transport services for years to come. In England and Wales, the census is planned and carried out by the Office for National Statistics. Elsewhere in the UK, responsibility lies with the National Records of Scotland and the Northern Ireland Statistics and Research Agency.

Title ONS and NHS Digital Census-Mortality-Hospital Episodes Linked Data Asset
Description The purpose of this dataset is to enable analysis of deaths involving COVID-19 by multiple factors such as ethnicity, religion, disability and known comorbidities as well as age, sex, socioeconomic and marital status at subnational levels. 2011 Census data for usual residents of England and Wales, who were not known to have died by 1 January 2020, linked to death registrations between 1 January 2020 and 20 November 2020 on NHS number. The data exclude individuals who entered the UK in the year before the Census took place (due to their high propensity to have left the UK prior to the study period), and those over 100 years of age at the time of the Census, even if their death was not linked. The dataset contains all individuals who died (any cause) during the study period, and a 5% simple random sample of those still alive at the end of the study period. For usual residents of England, the dataset also contains comorbidity flags derived from linked Hospital Episode Statistics data from April 2017 to December 2019.
