Sarcopenia, Frailty and Clinical Practice in Older People

Abstract
The mission of the MRC Lifecourse Epidemiology Unit is to provide a centre of excellence which uses epidemiological methods to promote human health by delineating the environmental and occupational causes throughout the lifecourse of: (1) chronic musculoskeletal disorders (osteoporosis, osteoarthritis and sarcopenia); and (2) metabolic disorders (diabetes mellitus, obesity and cardiovascular disease). Through understanding these causes, we aim to develop population-based and high risk preventive strategies against these disorders. This programme will explore the mechanisms which underlie the ageing of human muscle, and lead to physical frailty in later life.
Technical Summary
We will establish an ageing research programme that builds on our progress to date in the fields of sarcopenia and cognition, and capitalises on our expertise in these areas to investigate their links with frailty and wellbeing. Our aim is to carry out high quality interdisciplinary ageing research that informs clinical practice and translates into the improved health of older people. We will focus on mechanisms, lifecourse influences, clinical consequences and interventions. In a group of HCS participants, we will examine the contribution of altered muscle fibre composition, reduction in muscle fibre number and size, and reduced capillary density to the link between small size at birth and sarcopenia. Understanding the cellular mechanisms underlying the association between birthweight and sarcopenia has the potential to inform clinical practice in older people with regard to both prediction and prevention. As part of the HALCyon research programme, we will use data from nine UK cohorts to investigate influences over the lifecourse on cognitive and physical capability and psychological wellbeing in later life. We will also examine the longitudinal relation between sarcopenia and physical frailty and subsequent wellbeing and cognitive function. Study of the inter-relationships between cognition, wellbeing, sarcopenia and frailty across the lifecourse will enable causal associations to be identified and inform the timing of interventions to maximise capability and wellbeing in later life. We will carry out two studies to elucidate the consequences of sarcopenia, frailty and chronic disease for receipt of hospital care. Firstly, by linkage of the existing HCS data to routine Hospital Episode Statistics data, we will investigate the association between sarcopenia, frailty and chronic disease and incidence of hospital admission. Secondly, in patients aged over 80 admitted to the Acute Medical Unit in Southampton, we will examine how these conditions influence length of stay and adverse clinical outcomes. The findings from these studies have the potential to inform both clinical practice and health policy. Finally, we plan to develop two clinical interventions for sarcopenia and frailty. In the first study, of community dwelling older people participating in the Hertfordshire Physical Activity Trial, we will assess the short- and long-term effects of exercise training on sarcopenia, physical performance, frailty and wellbeing. In the second study, of hospitalised older people, we will investigate whether the use of volunteers to provide mealtime assistance can improve dietary intake and clinical outcome through an effect on these conditions. This programme will permit investigation of the mechanisms underlying the developmental origins of sarcopenia, identify inter-relationships between sarcopenia, cognition, frailty and wellbeing across the lifecourse, and determine the effect of these on the receipt of hospital care. The findings will provide evidence for the development of clinical interventions to reduce sarcopenia and frailty and improve the health and quality of life of older people.

Results and Impact | Cyrus Cooper was interviewed for an article that was published in the Telegraph as a result of the paper that was published in the BMJ on Vitamin D. His comments included: Cyrus Cooper, Professor of Rheumatology at the University of Southampton and President of the International Osteoporosis Foundation who contributed to the study said that bone density was probably some two to three times better at predicting someone's risk of getting osteoporosis than their level of Vitamin D level. "I don't think this paper debunks the idea that Vitamin D is useful", he said. "It's just that it is a small determinant. You'd do some good if you make sure that your Vitamin D levels are okay." Commenting on the current UK advice for management and prevention of osteoporosis he said: "The current guidelines are appropriate however a bone density evaluation needs to be done as part of fracture risk screening". |

The talks given over the course of the two days were around the burden of undernutrition and how disease can impact on nutrition and vice versa. A wide variety of talks were give; from my point of view participants, many of whom do not work with older people, gained insights into the common conditions of frailty and sarcopenia and how they impact on the older person. |

Results and Impact | On 22nd October 2021 Elaine Dennison wrote a news report on the Hertfordshire Cohort Study COVID-19 study which reported on the observations made on how the pandemic affected their study participants. Some of these findings were presentation at the Society for Social Medicine and Population Health Annual Scientific Meeting and at the Nutrition Society 2021 Summer conference. These both generated considerable interest. Although the findings highlighted the resilient nature of older adults in terms of being able to cope and adapt, it also emphasised their loneliness and isolation as well as fear of the virus and effects on mental health. Findings highlighted the need to consider this group when developing public healh interventions to support health and wellbeing, including strategies for improved and safe social contacts and physical activity. |

Results and Impact | The MRC LEU set up a stall with practical activities illustrating the sugar content of different products. Most people visiting the stand were parents with children. The children made a healthy eating pledge at the end of the activity and wrote it on a colourful board. Many people were surprised how much sugar is in certain products which can seem healthy. |

Results and Impact | On 23rd June 2018 a number of MRC LEU staff (Prof Cyrus Cooper, Prof Hazel Inskip, Prof Elaine Dennison, Prof Sian Robinson, Gregorio Bevilcqua, Leo Westbury, Barbara Fariborz, Martin Stevens, Tina Horsfall, Holly Syddall, Kate Ward, Shirley Simmonds, Millie Parsons, Michael Clines, Vanessa Cox, Clare Harris, Harnish Patel and Ilsa Bloom) travelled to Hertfordshire for the MRC Festival of Medical Research where they held the Hertfordshire Cohort Study Open Day at Harpenden Public Halls. There were two sessions, one in the morning and the other in the afternoon. Many of the participants of the Hertfordshire study, their children and grandchildren attended the event which included a talk by Cyrus Cooper and hands on activities - refreshments were provided. An article appeared in MRC Life and another article about the open day was sent to Charlotte Campbell at UCL to be included in the CLOSER newsletter. |

Results and Impact | Around 20 members of staff and one PPI member volunteered on 15th June to help at the Southampton Parkrun#364. They set up their stall, took grip strengths of some of the 1045 runners and 72 volunteers, which were recorded on a graph indicating grip strength, sex and age. This resulted in some healthy competition between those taking part. They also asked people what aspects of health they would like more research to go into and which were important to them. As an incentive to help the MRC we gave away hundrends of bananas, slap wrist bands, water bottles and mesh running vests. It was a very successful event and we plan to do it again. Other volunteers for this event included: Tina Horsfall, Sarah Shaw, Sue Curtis, Elena Zaballa, Julia Hammond, Chris Taylor, Gregorio Bevilacqua, , Sarah Jenner, Pam Mahon, and Phil Titcombe. Sarah received an email of thanks from Head Office which was followed up by a certicate to the MRC LEU for taking part. |

The aim of this was to provide insights to pupils about the kinds of scientific jobs available and to encourage interest in this area. I was one of the scientists at this event. The open day was extremely well attended and we had great interest in both the activities and also our research ongoing within the Unit, particularly among undergraduate students and their parents. People reported changes in their views and opinions and asked for further information. |

Results and Impact | "I'm a Scientist get me out of here", part of the MRC Festival Zone from 4th-29th June 2018 with over 1,300 students and their teachers taking part. This involved a 30 minutes live webchat where pupils were able to ask any questions they wanted and there was also the ability for students to ask further questions via the ASK function of the website. During the 4 week event Millie took part in 30 of the 30 minute webchats (15 hours in total) and then responded to a further 58 questions. The intention of the event is to encourage children to consider the importance of science and demonstrate the career options open to them in science. |

Parents and children attend this day-long event to take part in science-related activities and to encourage interest in this area. I was part of the MRC LEC team that hosted an activity which required the audience to grade posters produced by children about bone health. The audience reported a positive change in views and opinions after judging the posters regarding the importance of bone health. |

Results and Impact | This group is led by Pr Jean-Denis Laredo and organizes a meeting annually. Prof Cooper was invited to attend the meeting and present as part of a more general round table session where effectiveness of anti-osteoporotic treatments on the prevention of vertebral fractures was discussed. The title of Prof Cooper's presentation was "Epidemiology of osteoporotic vertebral fractures" (with a focus on the evolution over time of incidence and prevalence of vertebral fractures but also on the consequences of vertebral fractures). The participants of this meeting were represented by radiologists but also neurosurgeons, orthopedic surgeons, and few rheumatologists. |

The project, led by Jon Tobias, Professor of Rheumatology at Bristol Medical School, will involve a collaboration between researchers from the University of Southampton, as well as Bristol, Manchester, Aberdeen, Cardiff and Queensland Universities.
