Abstract
Physical activity is important for young people’s current and future health. However, it appears that many young people are not active enough to enjoy these health benefits. We also know that children do less physical activity when they become older, particularly during teenage years. Identifying ways to maintain and promote young people’s levels of physical activity is therefore important for public health. Physical activity behaviour is complex, not just consisting of sports activities but including behaviours such as walking the dog, cycling to school, playing in the playground and physical education lessons. How active young people are is influenced by many factors, such as their preference for physical activity, the provision of footpaths in the neighbourhood, weather and how much their parents support them in being physically active. How these factors work together in promoting physical activity and how to use this knowledge to promote increases in physical activity is still largely unknown. The Behavioural Epidemiology of Physical Activity Group therefore aims to improve the long-term health of young people by: - developing and evaluating interventions to increase physical activity in young people - increasing our understanding of where, when and how physical activity interventions in young people may be applied
Technical Summary
Physical activity during childhood is important for both contemporary and future health. Evidence suggests that young people’s levels of physical activity are insufficient for health. More importantly, physical activity on average declines with age. Identifying ways to maintain and, where applicable, promote physical activity in young people is therefore a key public health issue. Physical activity is a complex behaviour, influenced by a combination of factors from multiple ecological levels, including individual, socio-cultural, policy and environmental-level factors. The relative importance of these factors and how they interact is still largely unknown. Moreover, our current understanding is largely based on cross-sectional evidence, limiting our ability to draw conclusions about causality and inform intervention development. The two main aims of the Behavioural Epidemiology of Physical Activity Group are to: - develop and evaluate interventions to promote physical activity in young people - use observational research to further understand where, when and how physical activity interventions in young people may be targeted Intervention development and evaluation are a core part of the programme. Using a structured and iterative approach, we undertake explanatory trials to understand the feasibility and effectiveness of specific intervention strategies based on our previous observations. This approach allows for a greater understanding of the causal mechanisms behind behaviour change, and the potential effectiveness of specific components of future, more complex, interventions to be evaluated later. An example is the development and evaluation of a physical activity promotion intervention targeted at families, instead of the child alone. Here, we will use the intervention mapping approach to guide intervention development, and design a high quality evaluation including objective measurement of family-based physical activity. To inform future intervention development, we will continue to increase our understanding of physical activity behaviour, its influences and consequences throughout the different stages of childhood, with a predominant focus on prospective data. An example dataset we will use for this is the International Children’s Activity Database (ICAD). ICAD is an open-access dataset which aims to overcome comparability issues of single studies and currently holds data for over 30 000 children world-wide. We will both utilize the existing ICAD dataset as well as strengthen it by adding additional data to increase its utility for longitudinal analyses and the identification of influences on behaviour. In summary, this programme aims to develop and evaluate interventions to change young people’s activity behaviour through a thorough understanding of the patterns, consequences, correlates and determinants of the behaviour of interest. The work will lead to the evaluation of comprehensive physical activity promotion interventions at different stages of childhood with the aim of improving the long-term health of young people.

Description | The CASE active lessons intervention is a training programme for secondary school teachers to enable them to incorporate active elements into their lessons. This can take place indoors or outdoors and is to be built into their existing lesson plans. Description | The Families Reporting Every Step to Health intervention is a self-monitoring and goal-setting intervention targetting the whole family. It is delivered using an online platform on which families virtually travel across the world. Families wear pedometers and are encouraged to set goals to visit new countries/cities. Feasibility testing was conducted in spring/summer 2017, followed by further refinement and PPI testing. A refined version of the intervention will be tested in the FRESH pilot study, commencing Mar 2018. |

Title | GoActive |
Description | GoActive is a secondary school-based physical activity intervention. GoActive has been designed for all students, including those who are not usually interested in sport. During GoActive, students are encouraged to try new activities by teachers and mentors from older years in the school. Students gain points for trying new activities and points are entered into a competition between classes. The GoActive intervention was developed based on evidence and in collaboration with students and teacher. Successful feasibility and pilot testing was completed in 2014, and a fully powered cluster-randomised controlled trial is due to commence Sept 16. |

Results and Impact | Big Biology Day. Unit members participated in two IMS stands at the Big Biology Day event held at Hills Road 6th Form College. The first stand was a "Science festival" type stand with hands-on activities for families, for which the Unit provided the "Sugar Cube Challenge" and "Sweet Enough Challenge" activities which we had developed earlier. Unit members also contributed to a "faces of the IMS" booklet which gave examples of different career paths at the IMS, and which were handed out at a stand in the careers fair part of the event. The hands-on stand had over 250 visitors, and hundreds of CEDAR and MEC Epidemiology leaflets were handed out. (as well as most of our MRC Rulers and pens!) and queues to take part in activities for most of the day. There was a higher proportion of teens compared to the Cambridge Science Festival in March. There were fewer visitors to the stand at the careers fair, though still over 50 in the course of the day. Subsequently received request for work experience from 6th form student who visited careers stand. |

Results and Impact | "The Centre for Longitudinal Studies (CLS) is seeking views on the content of the Age 17 Survey of the Millennium Cohort Study (MCS). An open consultative conference will be held at the UCL Institute of Education on Monday 16th November 2015. It is open to all interested parties, including representatives of the MCS funders. Delegates from the scientific community, government departments, members of the third sector and other stakeholders will gather to discuss the proposed content of the data collection instruments at the Age 17 survey (MCS7). We are seeking audience views on the proposals which will be presented at the conference. The presentations will be available after the conference on the Centre for Longitudinal Studies (CLS) website by the end of November. We will have open discussion at the conference, which will continue via the CLS website until the end of December 2015. More than 500 visitors, including more than 300 children, attended our stand over the two days, and partivcipated in activitiies and talked with Unit staff. The response was very positive. We also gave away over 150 copies of a MRC Cambridge colouring book and flyers for the MRC Festival in June. |

Results and Impact | Erika Ikeda recruited speakers and managed online events using Zoom for 12 external seminars in 2020. The seminars were advertised by email to subscribers to the Unit events email list, on the Unit website, and through the Unit Twitter and Facebook accounts. Registration was required, but all were welcome to register. The audience for these seminars varied according to the topic being discussed, but usually consists of researchers & academics, policy makers and third sector organisations. Most seminars were attended by 50-100 participants. An online survey of participants conducted in autumn 2020 indicated that about two thirds were from outside the MRC Epidemiology Unit/CEDAR, and that a quarter were from institutions located outside the UK. |

Part of the "GoActive" programme of work we reached an audience of ~10 - middle school-aged children, this population being the specific target of "GoActive". This interactive event was designed to discuss possible ways of future contact to the target age group, to discuss the next measurement session incentives for participants, and to finalise the questionnaire (ensuring that the language used was appropriate for the age group). Information obtained during the session was used to inform the future actions of the GoActive study. |

Results and Impact | 115 researchers and policymakers attended the CEDAR conference entitled "What shall we do about diet and physical activity?" to discuss matters of policy and practice on October 9th 2013. Three group members (HEB, HLB and KH) each presented poster. 30% of attendees filled in evaluation forms. The overall feedback for the conference came back 65% "excellent" and 35% "good". Many positive comments about the combination of research & policy and quality of the discussion at the conference. Several respondents reported their intent to adapt their own research/policy work based on what they had learned during the event. |

Results and Impact | Dr Justin Guagliano assisted with the unit stand at the Cambridge Science Festival. About 150 children visited the stand and took part in activities, with a similar number of adults over 5 hours. About 70 families left suggestions fro lowering risk of diabetes in the population. We handed out about 100 copies each of the Great Snackington game and MRC Cambridge Units Activity book, as well as about 40 copies of Epigram, our annual newsletter, as well as several other leaflets including FEAT, Propensity to Travel and the MRC Epidemiology Unit trifold. The number of good quality conversations with stand visitors wseemed higher than previous occasions, with volunteers reporting that they were able to discuss the various approaches the Unit takes to studying type 2 diabetes and related disorders. Having activities (including Gt. snackington) that highlighted different areas helped these discussions. |
