Abstract
This proposal is to collect new information from families already taking part in the Born in Bradford (BiB) study, which was set up in 2007 to track the lives of 13,500 babies and their families. The aim of Born in Bradford is to find out more about the causes of childhood illness by studying children from all cultures and backgrounds as their lives unfold. Mothers joined the study when they were pregnant and we collected information from them and their partners about their health, their families and their social and economic circumstances. We have collected information on their babies' health and development at birth and as they grow up.
Now the BiB children are in primary school and we would like to ask them and their families some new questions. We plan to collect information from at least 9,000 BiB children at age 7-10 years and their parents (8,500 mothers and 5,000 fathers/partners). We will work with schools in Bradford to ask children and teachers to complete questionnaires. Children will use laptop computers to complete assessments and answer questions about their lives, health and wellbeing We will also ask parents and children to come to a 'BiB bus' or another community location to complete questionnaires and have some measurements. There we will ask families about changes in their social and economic circumstances, and about their health and wellbeing, and we will ask children about their diet and how much exercise they get. We will take blood samples from children and parents, measure their height, weight, body fat and bone density.
This information will be used to understand children's health and wellbeing, how they have changed over time and the causes of differences in childhood health and wellbeing. In particular, we would like to understand the factors that affect: (1) child social and emotional wellbeing; (2) children's cognition; (3) healthy growth in children and heart health in children and adults.

Technical Summary
This proposal is for a cohort-wide data collection sweep of Born in Bradford (BiB), a prospective pregnancy/birth cohort established to examine how genetic, nutritional, environmental, behavioural and social factors affect health and development during childhood and into adult life, in a deprived multi-ethnic population. We will conduct a full clinic and interview sweep of at least 9,000 BiB children at age 7-10 years (plus school-based neurocognitive assessments) and their parents (8,500 mothers and 5,000 fathers/partners). These newly collected data will be linked with all existing data and provide the first comprehensive follow up of BiB families since recruitment. Our objectives are to investigate: (1) the determinants of child social and emotional wellbeing at age 7-10; (2) the determinants of child cognitive and motor development in childhood; (3) the determinants of healthy growth in children and of adiposity and cardiometabolic health in children and adults. School-based child assessments will include cognitive assessments, a wellbeing questionnaire, and teacher-administered Strengths and Difficulties Questionnaire (SDQ). Community-based family assessments will include parent questionnaires and physical assessments, parent-administered SDQ, child diet and activity questionnaires and physical assessments. Multivariable regression analyses will be used to address study objectives. Anonymized data will be made widely available to academics, creating an international resource for exploring life course determinants of health, development and social wellbeing in a deprived multi-ethnic population.
Planned Impact
Our research will have wide impact for a number of key stakeholders. Our key beneficiaries will be national and international researchers, health and education professionals, policy and decision makers and families, and we are experienced in translating our findings into impact.
Academic impact
Our strength is in the integration of the best social and biomedical science, combined with innovative approaches to data linkage, to understand key drivers of healthy development and a healthy life course. Our future impact will reflect this strong collaborative and integrative approach across disciplines.
The addition of a new wave of data to the current BiB cohort dataset will create a valuable longitudinal dataset, adding biomedical and social data at age 7-10 to baseline data and on-going record linkage. We will facilitate the use of this resource by the wider academic community, both nationally and internationally. We are committed to data open access and to date have processed requests for data from external collaborators for 128 separate studies.
BiB will continue to contribute to a number of large European and international collaborations, pooling data in order to achieve high quality research with large sample sizes, and comparison of findings with cohorts in other contexts to generate internationally important policy-relevant findings. In the past, collaboration has led to impactful research on environmental pollutants in relation to development, growth and health, and the relationship between maternal socioeconomic position and preterm birth.
Economic and societal impact
BiB's new wave research will enable us to continue BiB's positive impact on health and wellbeing in Bradford. Our experience to date has shown our ability to engage with local health care providers and policy makers to influence effective health service delivery (see list of impact to date in Annex 4).
At a societal level, BiB facilitates knowledge and understanding of the factors that shape the wellbeing of children and families in a multicultural city. BiB has attracted considerable media attention and we will continue working with media organisations to disseminate findings widely to the public throughout the project.
The professionals involved in BiB so far have developed strong ownership and involvement in the study, and catalysed change in the care and services provided as appropriate. We will continue this approach, which will enhance skills for staff working with BiB and continue to improve services provided in Bradford.

Description | Continuous glucose monitoring (CGM) is used in research of diabetic patients and increasingly in 'healthy' cohorts. A large amount of data (e.g. interstitial glucose measures every minute for up to 6 days) but to date research using is inconsistent in which measurements from the CGM are including in research analyses which makes it difficult to pool studies and raised the concern that of many measurements that could be produced researchers select those that fit their hypothesis, or researchers may select outcome measures based on what they are familiar with (e.g. AUC or an early morning 'fasting' glucose) which does not make best use of the available data and may not provide the most accurate or rigorous results. We have developed and made freely available an easy to use tool (GLU) which produces a set of standardised measures from CGM and have disseminated it with the aim that researchers use all of the outputs in order that a catalogue can be developed of the type of measures that most reflect glucose variability in different contexts and so that results in this field can be pooled. We have piloted the tool in one study (ALSPAC) and are undertaking further analyses in cohorts and experimental studies |

Title | eQTM catalogue in children's blood |
Description | Background: The identification of expression quantitative trait methylation (eQTMs), defined as associations between DNA methylation levels and gene expression, might help the biological interpretation of epigenome-wide association studies (EWAS). We aimed to identify autosomal cis eQTMs in children's blood, using data from 832 children of the Human Early Life Exposome (HELIX) project. Methods: Blood DNA methylation and gene expression were measured with the Illumina 450K and the Affymetrix HTA v2 arrays, respectively. The relationship between methylation levels and expression of nearby genes (1 Mb window centered at the transcription start site, TSS) was assessed by fitting 13.6 M linear regressions adjusting for sex, age, cohort, and blood cell composition. Results: We identified 39,749 blood autosomal cis eQTMs, representing 21,966 unique CpGs (eCpGs, 5.7% of total CpGs) and 8,886 unique transcript clusters (eGenes, 15.3% of total transcript clusters, equivalent to genes). In 87.9% of these cis eQTMs, the eCpG was located at <250 kb from eGene's TSS; and 58.8% of all eQTMs showed an inverse relationship between the methylation and expression levels. Only around half of the autosomal cis-eQTMs eGenes could be captured through annotation of the eCpG to the closest gene. eCpGs had less measurement error and were enriched for active blood regulatory regions and for CpGs reported to be associated with environmental exposures or phenotypic traits. 40.4% of eQTMs had at least one genetic variant associated with methylation and expression levels. The overlap of autosomal cis eQTMs in children's blood with those described in adults was small (13.8%), and age-shared cis eQTMs tended to be proximal to the TSS and enriched for genetic variants. |

Results and Impact | IEU researchers have led online epidemiology sessions for students at two Bristol primary schools. Having outlined their own research, IEU researchers led the 11-year olds in devising their own epidemiology research questions about COVID-19 or the effects of the lockdown. Their questions included: Can eating chillies help prevent COVID-19? Can alcohol make COVID-19 worse? and Do people sleep more, less or the same in lockdown? The students then worked out how researchers might collect and process data to help them to work out the answers to their questions. Through so doing, they considered the challenges of working with incomplete data in rapid-response epidemiology. They also engaged with ideas of causality and the responsible communication of research findings. The 11-year olds greatly enjoyed the sessions, saying, "I enjoyed this experience because I learnt a lot about the process of how scientists study" and "It was good to meet scientists - I was inspired. |

Results and Impact | We the curious is a science museum in Bristol. We are doing a pilot project with them that will enable us to determine the youngest age (in children) at which it is feasible to capture data on capillary glycocalix through an imaging technology. The data assessment requires a probe to be placed under the child's tongue for 7-10 minutes to capture sufficient image data. We worked with We the Curious to agree the recruitment and data collection protocol. Older children / young adults (14-19 year old) will collect data and with our support do analyses. The results will provide the answer we need and all data will be destroyed once analyses are completed. We will use the results to decide the age groups in ALSPAC G2 and in other cohorts (e.g. in Bradford) in which we will collect these data. We the curious are keen to work on other projects with us and to build stronger links with the University for mutual benefit with respect to public engagement and involvement. This current project is the first project with them. Hopefully, it will lead to many further projects. |
