Abstract
The UK Dementias Platform (UKDP) is a radically new approach to dementias research, bringing together data from around 2,000,000 study participants from 22 cohorts to try and discover the causes of dementia and to find out ways of slowing it down. The platform has been funded to the level of £4.5M and invited to apply for a further £7.5M.
To achieve our goals of discovering the causes of dementia and finding out ways to slow it down, we will analyse data collected over many decades from throughout the UK to identify targets for drugs studies.
We will also enrich strategically selected cohorts to provide resources for dementias research in general. This will involve finding out how the early dementia processes begin, how our genes affect our risk of dementia and where our body goes wrong in maintaining and repairing itself.
There are many practical challenges involved in doing this work including knowing how to assess dementia in large studies where having everyone examined by a doctor would be prohibitively expensive. Another challenge is how to measure cognitive decline in the population as when people begin to decline they are frequently less motivated to do cognitive tests.
There are also important ethical issues surrounding this work and we have specific projects looking at ethical legal and social issues surrounding taking part in detailed measurement studies as well as in considering brain donation.
To help make an impact as soon as possible, we will work closely with industry partners who are best placed to turn basic scientific knowledge into effective therapies.
Even if we cannot cure dementia in the short term, it would count as a success if we found ways of slowing it down so that nobody need lose their dignity as they grew older.

Technical Summary
The UK Dementias Platform (UKDP) is a radically new approach to dementias research, providing a highly efficient and cost-effective translation pipeline from basic discovery through to early phase trials (Fig 1). By combining the analytic and statistical power of >20 clinical and population cohorts representing 2M participants and creating a large readiness cohort, UKDP will create an optimal environment for basic discovery activities relevant to the clinical progression and human impact of dementia. Although UKDP is intended as a strategic and sustainable resource generating increasing scientific benefit over time, it will also deliver short and medium term benefits through the analysis of existing data, the enrichment of strategically selected cohorts, developing new ways of working with industry, and generating interest and further funding for dementias research generally.
A single portal informatics hub (funded in stage 1) will be used for integrating data from participating cohorts and triangulating evidence between cohorts. The stage 2 proposal focuses on providing strategic resources for EM by enriching strategically selected cohorts, addressing key methodological issues and pump-priming a programme of EM studies.
Further development of the platform is anticipated as the opportunities it provides are more widely understood. For example, predicated on the ability of the platform's ability to provide accurately risk stratification, MRC and NIHR have recently funded a deep and frequent Phenotyping (DFP) feasibility study with a view to providing a further £5M for the full study. The DFP project may be considered a stage 3 development of UKDP and is an example of how UKDP can be used to leverage further resource for dementias research.

Our aim is to 1) base the science of the platform on the best information available, 2) create momentum in dementias research by being inclusive of, and synergistic with, other initiatives.
For industry, partnership discussions are already underway with exchanges of ides, interests and needs between academic and industry stakeholders. Industry have identified their need for access to conversion (early MVI to dementia cohorts and for experimental medicine studies to conform to regulatory requirements.
We remain committed to raising the profile of contemporary debate about dementia and its treatment. We wish to encourage a culture of commitment to solving this problem. By increasing awareness at all levels of society we intend to leverage resources for the platform and for dementia research in general, to increase awareness of the need for earlier interventions and better targeted treatment in general by health service providers and the public alike.
Engagement with the general public and with patients and carers is a very important part of our mission. This serves not only to communicate our research findings and their relevance but also to address such issues as stigma in society and the research culture in the NHS in relation to dementia and older people. In addition to using the platform web-site to communicate to the general public, we will also liaise with charities and advocate groups such as Age UK and the Alzheimer's Society to promote our work and findings and to engage them in shaping the work programme.
In addition we will have a dedicated free-phone number available 6 days a week and a communications officer at 50% FTE over 5 years whose responsibility is to develop and implement a communications and public engagement strategy.

Description | This report relates to Dementias Platform UK (DPUK) Objective 3 of Work Package 13: Brain Donation. Specifically, objective 3 is to develop agreed standards for the collection, classification and storage of brains tissue. Harmonisation of procedures across brain banks enables i) comparable standards of quality and diagnostic categorisation to be employed, ii) implementation of comparable sampling, preparation and storage techniques and iii) provision of consistent key tissue quality and pathology data. Consistent with the diversity of researcher requests, it is also necessary to preserve a sufficient degree of operational flexibility in accordance with the nature of the disease and user demand. In such instances, a rationale and explanation of any differences can be provided, or where appropriate, a view presented as to when convergence might be achieved in the future. The policies outlined below are those agreed by the 4 Medical Research Council (MRC) funded brain banks as part of a milestoned funding package and are used as a minimum protocol by all the banks in the MRC UK Brain Bank Network (UK BBN). Minimal standards for collection are presented in relation to i) Brain tissue, ii) Clinical and Neuropathological information and iii) Neuropathology (Classification & storage). |

Clarification of scientific questions and identification of what DPUK seek to learn from donated brains would help to shape a brain donation strategy. 8. Conditional support for a UK Biobank brain donation pilot study. 9. A decentralised model based upon the accommodation of extra DPUK activity within existing brain bank infrastructure across the 10 brain banks is favoured. 10. The long term suitability of alternative models for consenting, centralised collection and storage warrant exploration. |

Description | A briefing document on access to post-mortem samples and data from the proposed DPUK collection From the discussion above decisions concerning access to DPUK samples and data need to be taken at an early stage. A number of key factors determine the framework for granting access to tissue samples, accompanying neuropathological and clinical data through a DPUK platform. These factors relate to custodianship, ethical approval, eligibility for access, the application process and the conditions of access. Following specific DPUK guidance in relation to each of these factors, subsequent actions in defining a sample access plan include the development of 1) a standard operating procedure, 2) principles of access, 3) a common request application form, 4) an application process and 5) conditions of access. While a dual approach to access implies that established brain bank and cohorts procedures will cover many aspects of the request process, the section below highlights those elements requiring unique consideration in relation to DPUK generated applications. Title | Alzhemer's Disease Polygenic Risk Profiling |
Description | This model used data from the powerful dataset comprising 17 008 cases and 37 154 controls obtained from the International Genomics of Alzheimer's Project (IGAP). Alzheimer's disease (AD) Polygenic risk scores were generated for 3177 cases and 7277 controls (GERAD data) and tested whether the alleles identified to associate with disease in IGAP sample are significantly enriched in the cases relative to the controls in the GERAD sample. The disease prediction accuracy was investigated in a sample of 3049 cases and 1554 controls (for whom APOE genotype data was available) by means of sensitivity, specificity, area under the receiver operating characteristic curve (AUC) and positive and negative predictive values. The best prediction accuracy AUC = 78.2% (95% confidence interval 77-80%) was achieved by a logistic regression model with APOE, the polygenic score, sex and age as predictors. |

Results and Impact | Dr Pavandeep Rai was a judge in an Beyond Boundaries 2020 competition; an outreach event to schools in Oxfordshire. The University of Oxford invited schools to participate in an art competition for Oxfordshire state school students to create art inspired by research from Black, Asian and Minority Ethnic scientists. Students were asked to choose one of six scientists to create an artistic response to their research (e.g. painting, drawing, sculpture, animation, other artistic media) along with a short statement explaining the artwork. Due to COVID-19, there was a delay to announcing the winners. The winners are to be called in January 2021 and an exhibition of their work will take place online now. |

Results and Impact | More than 75 EMIF team members gathered in Budapest March 14-16, 2016 to not only share status updates on the ongoing work, but also discuss new joint AD/Metabolic scientific questions to be addressed using the EMIF framework. Overall, tremendous progress was shown for both research projects, with many more exciting results being underway and opportunities for further collaboration identified. In particular the overlap between AD and metabolic disorders, due to inflammation, such as in Diabetes Mellitus Type 2, has provided opportunities for shared work. |

Presentations from speakers covering all areas of the Dementia Platform and cohorts encouraged to join the platform. Engagement of early career researchers through a poster presentation competition. 37 posters were exhibited, some with interactive stands, and demonstrated the progress of work delivered through the DPUK Work Programme and Infrastructure Networks. Dr Carol Routledge discussed the opportunities available through the Dementia Discover Fund and Maria Carrillo, PhD, Chief Science Officer from Chicago gave her first impression of DPUK "The Dementias Platform UK and Alzheimer's Association have a lot of opportunity to collaborate in the future in my view and John Gallacher and his team have also agreed with that, and this is one of the reasons why we have been asked to listen in and help future thinking and brainstorm ideas that might come about with DPUK's established network." |

Results and Impact | Giving a talk on types and diagnosis of dementia at this half day meeting. The Medicine and Me series of meetings at the RSM aim to update on important conditions and gives a direct voice to patients and their carers to air their concerns on the impact of diagnosis, investigation and management. Physicians and surgeons and indeed all healthcare professionals continue to learn from and be inspired to greater efforts by our patients to improve care. The audience at this meeting will comprise of those living with dementia, their families, carers and advocates, representing about 70% of those present; and clinicians and researchers, representing not more than 30%. Clinicians will include doctors, specialist nurses and others involved in the treatment and care of those who have this condition |

Results and Impact | I was interviewed for an online course about the rarer forms of dementia, my interview related to the clinical features of frontotemporal dementia. I also performed the voice over and welcome to the course that appeared in the course trailer video. The course, due to start March 14th 2016 is a 'massive open online course' available at www.futurelearn.com/courses/faces-of-dementia. Over 12,000 people have registered for the course (as of 8th March 2016). |
