Nutrition, Development and Lifelong Health: Studies in Developing and Transitioning Populations

Abstract

The mission of the MRC Lifecourse Epidemiology Unit is to provide a centre of excellence which uses epidemiological methods to promote human health by delineating the environmental and occupational causes throughout the lifecourse of: (1) chronic musculoskeletal disorders (osteoporosis, osteoarthritis and sarcopenia); and (2) metabolic disorders (diabetes mellitus, obesity and cardiovascular disease). Through understanding these causes, we aim to develop population-based and high risk preventive strategies against these disorders. This programme will explore the contributions made by maternal undernutrition to the origins of later diabetes and heart disease in the offspring. It is based in India, and includes a series of observational studies and trials.

Technical Summary

Past research in this programme has shown that the maternal undernutrition, and low birthweight and infant weight gain typical of poor rural populations in India are associated with reduced lean mass, bone mass and cognitive function in childhood and/or adult life, and an increased risk of low birthweight and stunting in the next generation. When early life undernutrition is followed by rapid childhood and adolescent weight gain or excessive adult weight gain, features that are enabled by economic transition and urbanisation, there is a high risk of insulin resistance and metabolic syndrome, type 2 diabetes and CVD. In women, this transition increases the risk of gestational diabetes, which also causes insulin resistance and diabetes in the next generation. Poor diet quality, leading to inadequate intakes of a range of vitamins and minerals, is common among poor rural and urban women, and is associated with lower birthweight. Vitamin B12 deficiency is common in India for cultural reasons, and may be a particularly important nutritional deficit in this population. Maternal vitamin B12 deficiency is associated with low birthweight, insulin resistance in the children, and increased maternal adiposity and gestational diabetes. A major focus of our future work will be on intervention studies. Of several possible intervention points in the dual cycle of risk, we have chosen the adolescent and young adult age group, with the objective of improving the health of young women before pregnancy, improving pregnancy outcomes and reducing metabolic/CVD risk in the children. Interventions will be designed not only for overtly undernourished women, but also for middle class urban women because they too, and their children, are at risk. Alongside these intervention studies, we will continue to follow-up our existing birth cohorts, to better define associations between early life factors and risk markers for later disease, disease endpoints (CVD and T2DM), bone health and cognition. In the Delhi study, we will explore inter-generational associations for these outcomes, by studying children of the cohort. Like the other ERC programmes, we will utilise the intervention and cohort studies to explore epigenetic mechanisms as a possible link between early nutrition and later outcomes. Other international collaborations will include studies of social factors in early life in relation to adult health and disease, and studies of the economic impact of impaired early development.

Results and Impact We held a workshop event with students (and their teachers) of year 7 and 8 at a public school in Mysore, India where we discussed aspects of nutrition, sexual and reproductive health, and lifestyle including addictions. There was also a question and answer session with the students asking us various queries and raising issues for discussion. After further discussion with the school principal and head teacher, we will carry out such activities on a rolling basis (annually). We also have plans to engage with the students to obtain their views on health education in schools and how it can be tailored to their needs.

The main aims were to seek their views on: 1) the information provided to them about the research for consent; 2) how they would like feedback from the results of the study; and 3) their interest in continued participation in the results. Our team presented the summary of the research followed by an interactive session. The response from the participants was very positive and they expressed satisfaction with the information provided and that they would like to continue to participate in the research.

Results and Impact The University of Aberdeen unit hosted a workshop with participants from the University of Aberdeen, Centre for Cellular and Molecular Biology, Hyderabad, Narayana Hospitals, Mysore, CSI Holdsworth Memorial Hospital, Mysore and the MRC LEU, Southampton. The main aims were to 1) share knowledge on brain imaging data analysis and data management and security; and 2) develop a grant application for the MRC Mental Health GCRF. We have submitted a grant application with Alison Murray and Kalyanaraman Kumaran as lead PIs.

Following that workshop, we have obtained joint funding for a pilot study in neuroimaging and for public engagement activities with our existing cohorts.

Results and Impact The Healthy Life Trajectories Initiative (HeLTI) consortium engaged in a discussion with invited expert scientists to maximise the potential from the set of harmonised HeLTI studies using the HeLTI platform. This engagement was facilitated by the Canadian Institutes of Health Research. The main aims were to discuss and develop new research ideas using the HeLTI platform which could then be converted into collaborative funding applications to funders. The HeLTI leads presented the platform, individual studies and work progress. The rest of the meeting involved discussions and development of new research ideas. Some new ideas which could result in potential grant applications may arise from the workshop.

This meeting was attended by funders and policymakers of the national funding agencies in India, Canada, China and South Africa and the WHO. The meeting was also attended by lead investigators of the HeLTI projects of all the sites. The main aims were to 1) update the funders on our progress with the project; 2) update the data sharing and governance arrangements; 3) exchange information and share knowledge/lessons learnt from studies; and 4) discuss the renewal of the grant. Covid-19 had a significant impact on the all the four studies and there was discussion around its impact in terms of costs and time.

Results and Impact My colleague Dr Krishnaveni and I were invited to Mount Litera Zee School (a private school) in Mysore to interact with students of Years 9 and 10 about a career in research. We gave short talks about our own research paths and then had a question and answer session with the students. The students were interested in research and asked us a number of questions about a career in research. The report of the event was also publicised in their school newsletter.

Results and Impact We held an engagement meeting with community members, local leaders, health workers, local government members, and patient representatives at B Matakere village. The meetings took place at the primary health care centre and the local tribal colony central area. The main intention was to engage with the local community about the complex intervention study planned in that area. It was an opportunity for our collaborators and funders to see the area and participate in the community engagement. The community is supportive of the project as they can see the potential benefits for their future generations.

Results and Impact The Healthy Life Trajectories Initiative (HeLTI) consortium engaged in a discussion with the Wellcome Trust on funding additional/mechanistic studies using the HeLTI platform; this engagement was facilitated by the World Health Organisation. The Wellcome Trust responded by hosting a meeting where they invited selected internationally renowned scientists to a meeting/workshop in London, along with the HeLTI principal investigators. The main aims were to discuss and develop new research ideas using the HeLTI platform which could then be converted into collaborative funding applications to the Wellcome Trust and other funders. The HeLTI leads presented the platform and work progress. The rest of the meeting involved discussions and development of new research ideas. It looks as though there may be at least two resulting grant applications arising from the workshop.

Results and Impact KK was the main organiser of the annual Healthy Life Trajectories Initiative (HeLTI) Meeting at Mysore in December 2019. This meeting was attended by funders and policymakers of the national funding agencies in India, Canada, China and South Africa and the WHO. The meeting was also attended by lead investigators of the HeLTI projects of all the sites, and additional investigators from India. The main aims were to 1) update the funders on our progress with the project; 2) review the data sharing and governance arrangements; 3) exchange information and share knowledge/lessons learnt from the pilot work.

It was aimed at postgraduate medical doctors/residents to support them in developing their research protocols. The course was held over two days in September and consisted of lectures and interactive sessions. This course is now likely to be an ongoing annual activity as the course was well received and we have been asked to repeat this for the next batch of postgraduate doctors.

Results and Impact There was a newspaper article in the Times of India entitled "How the mother of all medical studies began in Delhi's Iajpat Nagar". The study started 50 years ago with 8,181 babies and now spans three generations and provides vital clues about infant mortality, nutrition. The article mentioned Dr Bhargava who will hand the NDBC study over to Dr Sachdev at the end of the year. The study was also mentioned on three websites (Times of India, reddit.com and Indian Council of Medical Research)

The radio programme covered the rationale for the study, the details of data collection and engagement, and ethical implications. This activity helped with the engagement process for the study.

Results and Impact We held a training event for non-clinical staff on Covid-19 just as Covid-19 was beginning to rise in India. At that time, there were no cases diagnosed locally yet. The training was intended to make staff aware of Covid-19 and the basic precautions and preventive measures that needed to be in place to tackle it. This training was repeated on multiple occasions and was also recorded and made available to staff who missed the training.

Results and Impact We held a training event for specialist clinical staff (ICU staff, casualty staff etc.) on Covid-19 just as Covid-19 was beginning to rise in India. At that time, there were no cases diagnosed locally yet. The training was intended to make staff aware of Covid-19, its potential implications and the precautions and preventive measures that needed to be in place to tackle it. This training was repeated on multiple occasions and was also recorded and made available to staff who missed the training.

Results and Impact We held a training event/workshop event on the basic principles of ethics in medical research for our field staff. The main aim was to ensure that all staff were aware of ethical implications of biomedical research and that appropriate information would be provided to potential participants while obtaining consent. We intend to deliver this training to all new staff before they commence field work and to hold regular update sessions.

Results and Impact The workshop was organised by the Indian Institute of Population Studies (IIPS), Mumbai. The workshop focussed on lessons learnt from longitudinal studies with invited speakers (of whom I was one) followed by discussions. This was held to mark the launch of a new longitudinal study by the IIPS. The event received positive feedback from the study team and attendees.

Results and Impact My colleague Dr Krishnaveni and I were invited to participate in a workshop by Dr Chandak at Hyderabad to discuss future research projects and also for us to train his team and others at their institute (mostly PhD students) on research methods. The teaching was interactive and well received. The discussions with his team resulted in the development of a project to investigate genetic determinants of vitamin D on the Parthenon Cohort in Mysore. The study received intramural funding from the Centre for Cellular and Molecular Biology.

Results and Impact I was invited to deliver a workshop by Dr Rekha at the Swami Vivekanada Youth Movement, a large NGO in Mysore. They offer attachments to international students doing Masters degrees in public health. The attachments cover a range of public health issues specific to India and also socio-cultural aspects. I was asked to cover social determinants of health. The session was interactive and the students who were from the University of Minnesota appreciated the opportunity to develop a better understanding of social influences on health in India.
